Introduction
Airbnb is an online community marketplace providing a platform to rent or lend accommodations. This report will analyse said company, an emblem of the sharing economy, by first explaining its history and business model. Then, it will present the impact of Airbnb on relevant industries and communities in cities and how one can regulate the platform to manage its effect. Finally, it will detail future plans and risks of failures for Airbnb.
History and Business Model
Airbnb was founded by Joe Gebbia and Brian Chesky in 2007. The duo had the idea to rent some air mattresses in their home to pay rent. They would target people that couldn’t find any hotel due to market saturation (they lived in San Francisco). When they realised the potential of this idea, the duo called in Nathan Blecharczyk to join them. Together they developed the website and launched it officially in February 2008. The first goal of Airbnb was then to offer accommodation during big events where hotel rooms would be scarce. In 2009, after joining the Y Combinator, they expanded the website to have listings of rooms, entire houses, apartment, etc. That is when Airbnb found its business model and, in 2010 the company started to grow rapidly. Since then, over 400 million people used the platform to get accommodation (Airbnb, 2018a). Nowadays, Airbnb has over 5 million listings on its website in 81,000 cities across 191 countries (Airbnb, 2018a).
Airbnb sees itself as “a social website that connects people who have space to spare with those who are looking for a place to stay”. This idea is simple yet it disrupted a whole industry. Airbnb’s on-boarding is easy. They are two categories of users. The first one is the hosts. They are the people putting the listings on the website. It can go from simple rooms in your home to entire flats or more extravagant, igloos or treehouses. The second category of users is the guests. They are ordinary people looking for a place to stay. Airbnb takes fees on each reservation: a 6% to 12% fee from the guest and a 3% fee from the host for credit card processing. Recently, Airbnb launched some new services: experiences and places. For the experiences, hosts can share a local activity or leisure for a certain price. For places, guides can help tourists visit certain touristic sites. As we can see, Airbnb provides a service that didn’t exist on a large and global scale before. This leads Airbnb to develop a “Blue Ocean” strategy (Kim and Mauborgne, 2005), where a company creates a new demand in an unchallenged sector.
Airbnb is a model in the sharing economy. Kaplan and Nadler (2017) find three traits to a company in the sharing economy. The first one is use of advanced technology. Airbnb satisfies this trait as it developed powerful software to host all the listings on its website and fulfil the need of millions of travellers with the help of targeted recommendations. The second trait is for the company to be operating in parallel to a well-established industry. This is clearly the case with Airbnb competing against traditional hotels. The last trait is for the company to be running in a grey area of the law. Airbnb does meet this trait as we will see, later on, its relationship with legislative authorities. We can notice that those traits are not enough to define a company in the sharing economy. Constantiou, Marton and Tuunainen (2017) had a different approach to define the key attributes of a company in the sharing economy. They found three attributes: “access over ownership”, “peer-to-peer” and “allocation of idle resources”.
The last two attributes fit perfectly Airbnb as it is only a platform so it facilitates exchanges between people (peer to peer) and the concept of renting one’s home when not in use corresponds to the core philosophy of Airbnb (allocation of idle resources). Those two concepts have a different approach but taken together provide a good overall of the sharing economy’s main idea. Moreover, Constantiou, Marton and Tuuainen (2017) found that they are four models in the sharing economy. Those models are described using two factors, the “rivalry between the platform participants” and the “control exerted by the platform owner”. Applying this model, we can characterise Airbnb in the “chaperones” model where there is a high rivalry between platform participants and a loose control by the platform. Airbnb only gives recommendations to hosts regarding the way properties should be managed and as the hosts fix the price, there is strong competition between them to attract guests.
The Impact of Airbnb
They are some controversies on the impact of Airbnb on industries. The two main industries Airbnb affected the most are the hotel industry and the real estate industry.
Most complaints arose from the hotel industry. This industry is highly regulated and segmented. When Airbnb started growing, it challenged this industry who had not evolved in the past decades. As of 2018, Airbnb with close to 5 million listings worldwide is far ahead of the biggest hotel group, Marriott who has only 1.2 million rooms globally (Marriott, 2018). The hotel industry felt threatened and started to complain about this new actor disrupting their business. The main complaint about Airbnb is insufficient regulations compared to the heavy legislation concerning hotels (Sickel, 2017). Hotels then claimed Airbnb was unfair competition and was impacting their revenues. However, a study discovered that in Texas, an appreciation of 1% in Airbnb listings only reduced hotel revenues by 0.05% (Zervas, Proserpio and Byers, 2017). Those numbers seem to show Airbnb doesn’t have a significant impact on the industry. Nevertheless, the study found the main hotels impacted by Airbnb were low-end hotels and particularly hotels without business conveniences. This study then tried to determine if any regulations on Airbnb would impact the hotel revenues and observed it was insignificant.
In a study on the impact of Airbnb on real estate, Barron, Kung and Proserpio (2018) found Airbnb has a small impact on housing prices and rental rates. They explain this impact on rental rates because landlords switch from proposing long term rentals to offering short-term rentals. The rise of rental rates causes an augmentation of housing prices as well as the ability to generate revenue from extra housing capacity. Furthermore, in a study by Quattrone et al. (2016) on the beneficiaries of Airbnb in London, the researchers found Airbnb is not necessarily impactful on the real estate industry. While, on the supply side, listings were available throughout the city, demand would still be concentrated in central London. This shows the most desirable Airbnb listings are in neighbourhoods already prone to inflation due to their localisation. Airbnb in this context impacts more the hotel industry, established in traditional touristic areas than the real estate industry.
To overcome the possible negative impacts of Airbnb on those industries, local and national authorities are starting to develop an array of legislation for Airbnb. Most big and touristic cities in Europe and in the United States started to roll out measures to regulate Airbnb. For example, Paris capped the numbers of nights one host can rent his home to 120 (Mairie de Paris, 2018). London (Deregulation Act 2015) and Amsterdam (City of Amsterdam, 2018) followed by capping to respectively 90 and 60 nights a year. Other major cities implemented different measures with the same goal to slow the expansion of Airbnb (New York, Berlin, Barcelona, San Francisco…). Nevertheless, cities are torn between restricting Airbnb - thereby slowing its expansion to satisfy the discontent of local actors and welcoming the economic benefits of Airbnb.
According to Airbnb (Airbnb, 2018b), guests in Airbnb accommodations stay 2 times longer and spend 2 times more than typical visitors. For example, Airbnb claims to have a positive impact on the French economy in 2016 of 6.5 billion euros (Bianchi, 2017). To solve this problem, Quattrone, et al. (2016) propose to use the idea of Miller (cited in Quattrone et al., 2016) where short-term rentals are legalised through “transferable sharing rights”. Those rights would give an individual the right for a certain period to rent his home. Quattrone et al. expand on the idea of Miller by proposing to create “algorithmic regulations” where regulations are adapted depending on real-time demand. For instance, authorities could decide to only permit renting during a specific time frame, holidays for example. Quattrone et al. propose this idea of adaptive regulations because they found that attractive neighbourhoods to Airbnb guests might evolve and change over time, and authorities don’t have the tools to adapt as quickly.
The proposition is to answer the main problem that authorities are facing, Airbnb entrepreneurs. Airbnb allowed the development of new entrepreneurs who would purchase entire blocks of flats or house to list them on Airbnb. Those entrepreneurs are the scourge of municipalities. They are the one responsible for the desertification of the city centre. To take an example, in Paris, the deputy mayor for housing, Ian Brossat said in an interview (Henry, 2018) that Airbnb induced a lost of 20,000 housings in the city centre. To back his claim, he said 26% of accommodations in the Ie, IIe, IIIe and IVe arrondissements are now not inhabited by Parisians. In their paper, Quattrone et al. recommend having a platform for the transferable sharing rights where people could sell their right to others if they do not use it. This would then not solve the problem of Airbnb entrepreneurs as they would still be major players but at least city council would have some control over them and could with real-time regulations restrict the rights in certain neighbourhoods as they see fit.
Future Plans and Risks
Airbnb has big plans for the future. The company unveiled in February 2018 a roadmap to get to 1 billion guest stays (Airbnb, 2018c). The plan includes a diversification of properties to rent with the addition of four new property types. Airbnb is launching a new service called “Collections”, designed to find a place to fit special occasion like honeymoons, weddings, group getaways, social stays… Airbnb also launched two new tiers, “Airbnb plus” and “Beyond” aimed at wealthier and more demanding guest. Finally, Airbnb is going to expand its 'superhost' program and create a new guest membership program to reward its most loyal customers.
While Airbnb seems on the path of future growth and expansion, it is not completely protected against failures. Tauscher and Kietzmann (2017) analysed a multitude of companies in the sharing economy to find causes of failure. Some causes are relevant to Airbnb: “low customer lock-in”, “low control over service quality” and “unexpected changes in the legal environment”. The first cause relates to the low switching cost for either guest or host to change platforms if a strong competitor of Airbnb would emerge. The second one is relative to the loose control of Airbnb on its hosts. Although as explained in the roadmap, Airbnb wants to tackle this problem by developing programs to standardise more fully its offer. For the last cause, the increasing regulations arising in various cities around the globe could seriously curb Airbnb’s growth.
Conclusion
Airbnb is now a major player in the hospitality industry. With a valuation of 30 billion dollar and generating profits, Airbnb has the power to fuel its expansion and accomplish its vision for the future: focusing on end-to-end travel experiences. Even though the company faces some controversies from competing industries, local communities and regulators, Airbnb can still find a way to work and grow with those affected by its service.

In real estate and property studies, the hedonic regression (P = f1, f2, f3, ..., fn) is often used to study the impact of a number of factors that affect housing prices [1] In economics, hedonic regression or hedonic demand theory is a revealed preference method of estimating demand or value. It breaks down the item being researched into its constituent characteristics, and obtains estimates of the contributory value of each characteristic. This requires that the composite good being valued can be reduced to its constituent parts and that the market values those constituent parts. Hedonic models are most commonly estimated using regression analysis, although more generalized models, such as sales adjustment grids, are special cases of hedonic models. Hedonic models are commonly used in real estate appraisal, real estate economics, and consumer price index (CPI) calculations. In CPI calculations, hedonic regression is used to control the effect of changes in product quality. Price changes that are due to substitution effects are subject to hedonic quality adjustments.
All in all, Hedonic regression is a revealed-preference method used in economics to determine the relative importance of the variables which affect the price of a good or service. These factors are determined using regression analysis.

Most real datasets consist of missing values, thereby requiring one to identify the missing values, to determine the extent and type of missingness, and to choose a course of action accordingly. Although a rich literature exists on data imputation, it is monopolized by an explanatory context. In predictive modeling, the solution strongly depends on whether the missing values are in the training data and/or the data to be predicted. The proposed solution is to estimate multiple 'reduced' models, each excluding some predictors. When predicting an observation with missingness on a certain set of predictors, the model that excludes those predictors is used. This approach means that different reduced models are created for different observations. Although useful for prediction, it is clearly inappropriate for causal explanation. However, it works in a hedonic model.
In biased parameter estimates, endogeneity can occur due to different reasons. One is incorrectly omitting an input variable, say Z, from f when the causal construct Z is assumed to cause X and Y. In a regression model of Y on X, the omission of Z results in X being correlated with the error term. Endogeneity can arise due to other reasons such as measurement error in X. The methods of detecting endogeneity are constructing instrumental variables and using models such as two stage-least-squares (2SLS).
Pros: The hedonic pricing model has many advantages, since this method of valuation can require a strong degree of statistical expertise and model specification, following a period of data collection. This model includes the ability to estimate values, based on concrete choices, particularly when applied to property markets with readily available, accurate data. At the same time, the method is flexible enough to be adapted to relationships among other market goods and external factors. The values of properties are broken down into components. With regards to valuing properties, a hedonic pricing model is relatively straightforward as relies on actual market prices and comprehensive, available data sets.
Thus, I believe the company should use the hedonic model.

I change logvalue to value because we need the actual value to see the hedonic model's accuracy. Although Logs turn multiplicative models additive, and they neutralize exponentials, they are nevertheless 'linear in logs.' In addition to turning certain non-linear models linear, they can be used to enforce nonnegativity of a left-hand-side variable and to stabilize a disturbance variance. I transfer every variable to logs because, such as 'unisf' into log(unitsf) because I think according to the model, logs can mostly make significant factor stable enough to determine value. Stata15 automatically selects 25 explanatory variables from 30 explanatory variables, and stepwise regression estimation removes the five variables of lnnumair, exclus, lnnumsew, lnkitchen and wash.Forward stepwise and Backward stepwise retain the same variable, so stepwise regression estimate result is more reliable.
Starting from your model with at least 30 variables use the stepwise command in STATA to perform both backward and forward model selection. Estimate the models on the training data. To better and more elaborate describe the marginal effect of each variable, an additional an better strategies to help to predict more precise home values would be benefits. however, I do not mean Hedonic is not good enough, it just not so concise and the logvalue would make trouble for the costumers to read. Costs of this decision would be the time, money spend on the new prediction models while the benefits of this decision would be more precise money can reduce unnecessary risk and lower the price to attract customers. In addition, the company can see more clearly how the variables affect the housing value and which ones are most significant and focus to build housing estate according to the models.

Introduction of Economic Slowdown
Economic slowdown is a business cycle and it’s generally occurring when there is a widespread drop in spending. Many time it’s happen when government issue new bill or act in the country which direct affect to the country’s GDP.
Economic slowdown started in india after the 2016-17 because in the first quarter of 2016-17 india’s registered GDP was 9.4% which is the sign of development but after the first quarter of 2016-17 GDP was fallen frequently.
As per Raguram Rajan statement many reasons behind the recent slowdown but 'ill-conceived demonetisation and the poorly executed GST roll-out' are the two key reasons behind the slowdown.
India has a long-standing problem of not collecting enough taxes given the size of its gross domestic product. Not only is it much lower than developed countries with comparable GDP sizes, it is below even comparable developing countries. India’s tax-to-GDP ration lags behind even Nepal.
The spurt in instances of job losses from automobile manufacturers to biscuit makers has led to the general acceptance of the downturn. This is the third instance of an economic slowdown for India in the past decade after the ones that began in June 2008 and March 2011. The technical term for the same is growth recession. A recession is defined in economics as three consecutive quarters of contraction in GDP. But since India is a large developing economy, contraction is a rarity. of negative growth for India was in 1979. A growth recession is more commonplace where the economy continues to grow but at a slower pace than usual for a sustained period, what India has been facing nowadays.
According to global broking firm Goldman Sachs, as of June 2 as of June 2019, the current slowdown has lasted for 18 months More than half of the decline in economic activity has been driven by a consumption slowdown, which appears to be broad -based, with components other than auto contributing more than twice the effect of autos to the total consumption decline.
Consumption, one of the key growth drivers of the economy, is on the slow lane. Though the slowdown started in high-ticket segments like real estate, auto and consumer durables thanks to the credit squeeze triggered by the NBFC crisis, it has now spread to other sectors. Consequently, the GDP growth rate for the first quarter of 2019-20 fell to a 6-year low of 5%. There was more bad news for investors. Experts say this slowdown will continue for some more time.
Above table shows the economic slowdown starts from 2016-17 and which shown after the first quarter of 2016-17 the GDP was fallen frequently and in the second quarter of 2019-20 the lowest GDP rate in over six years which is slipped to 5%.
Because of the economic slowdown the five sectors are affected most and that sectors are FMCG, AUTO MOBILE, STEEL, CONSUMER DURABLES, REAL ESTATE. This sectors situation is very critical because of the economic slowdown.
FMCG : When some of the biscuit makers talked about job losses due to fall in demand, the entire nation was taken aback. However, it turns out that the situation is not that grim and the entire episode was more about getting a GST rate cut for the sector. Nevertheless, companies such as Hindustan Unilever, Dabur India and Britannia Industries, among others, witnessed fall in volumes in the June quarter. These companies are mainly affected due to a fall in rural demand. While several companies in this segment Bata, HUL, Marico, etc—have beaten slowdown blues or reported limited impact, most continue to be highly priced and therefore, don’t offer much upside potential from current levels
AUTO MOBILE : The much-anticipated cut in GST did not come for the automobile sector during the meeting of the GST Council in Goa last week, leaving manufacturers to fend for themselves. The sales of automobile companies have been declining for at least 13 months. While a slowdown in the economy is the prime reason for falling auto sales, other factors such as the country’s transition to BS VI norms and push towards electric vehicles have further affected sentiments. The share price of heavy commercial vehicle manufacturers like Ashok Leyland and Tata Motors fell 50% and 56% respectively during the past year. Though prospects of these Companies are linked to overall economical cycles, analysts expect a revival in the CV industry during the second half due to pre-buying before BS-VI norms are introduced.
STEEL : High iron ore prices and weak demand have forced many steel makers to cut production as much by 50% in some cases. The steel sector also feels the pinch when auto makers go slow as this sector is the biggest client. In July, for the first time since December, 2017, steel prices went below INR 40,000 per tonne, making it unviable for steel manufacturers.
CONSUMER DURABLES : If FMCG is affected, how can consumer durables remain far behind. Sale of televisions, washing machines and other white goods have gone down just before the festive season. Makers of consumer durables have are clamouring for cut in GST rates. As a reprieve, the government last week scrapped import duty on open cell LED panels which accounts for 60-70% of the cost of televisions. This segment is a mixed bag and there is no pressure on low priced consumer durables and electrical appliances. For example, light electrical divisions of Havells, Crompton Greaves and V-Guard have grown by 24%, 16% and 18% respectively. However, most companies are facing issues in the industrial segments like cables, switch gears, etc due to ongoing troubles in real estate. However, the prospects of companies like TTK Prestige, which caters mostly to the retail segment through its kitchen appliances.
REAL ESTATE : The housing real estate market was already under pressure as non-delivery of projects became a huge menace and the government had to step in. With RERA rules applicable in various states, builders are now finding it tough to lure customers as penal provisions have become stiff. With the slowdown kicking in, consumers are uncertain about future income and are reluctant to make buying decision just yet. Number developers plunged about 46% between 2011–12 and 2017–18 and hence, we envisage that the share of top 10 developers in housing demand across major cities to touch around 50-60% (from 20-40% currently) over the medium term,” says a recent Edelweiss Finance report. In other words, the worst is already over for organised players. Most strong players like Godrej Properties are already pricing in these positives and therefore, don’t offer much upside from current levels.
Introduction of Unorganized Gold Market
India's gold demand slumped 32 per cent to 123.9 tonnes in the September quarter as higher prices and economic slowdown reduced the appetite for the yellow metal, according to a report. And this directly affect to unorganized retail market. The country, which is the second largest consumer of the yellow metal after China, recorded a 66 per cent fall in gold imports to 80.5 tonnes during Q3 of 2019 against the year-ago period. The decline in import was more steep as jewellers met their demand with old imported stock and recycling, the Gold Council (WGC) said Tuesday. In domestic market, gold prices had peaked to Rs 39,011 per 10 grams in September and are now ruling at Rs 38,800 per 10 gram.
During the first nine months of 2019, the country's cumulative gold demand declined to 496.11 tonnes from 523.9 tonnes during January-September period of 2018. The 2018 full year gold demand stood at 760.4 tonnes, the report said. Similarly, the cumulative gold import declined to 502.9 tonnes in the first nine months of 2019 from 587.3 tonnes in the year-ago. During the full year of 2018, India's gold imports had stood at 755.7 tonnes.
In India, there was decline in gold demand due to two reasons. One was sharp increase in prices by 20 per cent towards the end of Q2 and in Q3. The second reason was that, in various countries including India and China, general economic slowdown affected the consumer sentiment,' industry group WGC India Managing Director Somasundaram PR told .
Gold demand fell 32.3 per cent to 123.9 tonnes (including jewellery demand of 101.6 tonnes and bar/coin demand of 22.3 tonnes) during Q3 of 2019 when compared with 183.2 tonnes in the same quarter 2018, he said after releasing the report. Stating that gold imports too fell due to high prices and lower rural demand, Somasundaram explained, 'When demand is low, people recycle gold. Recycled gold in India rose to 90.5 tonnes in the first nine months of this year, as against 87 tonnes during the full year of 2018. This is going to be one of the highest year of recycling.
In terms of consumer sentiments, I don't think things have improved a lot because rural anxiety still continues. While demand increased during Diwali and Dhanteras, we don't think it will be better than last year, some wedding buying will happen and it is picking. It will not be higher than last year because high prices, he said.
Zaveri is not glittering. Its dilapidated buildings and narrow, congested lanes look forlorn, bereft of bustle. Very rarely do you see large families stoic men leading groups of happy women flocking jewellery shops to buy gold for an impending wedding. This year has been particularly bad for many jewellers in the area. Arguably the most valuable gold hub in India, home to over 5,000 jewellery shops, the 150-year-old Zaveri Bazaar has seen a mammoth fall in sales this marriage season, say oldtimers and traditional jewellers. It’s not alone; it’s a national trend.
Others traditional jewellery hubs Thrissur and Kozhikode markets in Kerala, Bow Bazar and Bagree Market in Kolkata, Johari Bazaar in Jaipur and Sadar Bazaar and Hauz Khas in Delhi are all marking decline in sales over the past few years. This is despite India continuing to import record bullion.
“Close to 35,000 marriages took place across 5-6 muhurats in November and December. Despite so many marriages, we did not see a significant spike in gold demand,” rues Kumar Jain, vice-president, Mumbai Jewellers Association. This wedding season has close to 85 muhurats or auspicious marriage hours, jewellers clarify.
Between September and June, jewellers in leading micro-markets hold 70-90 tonnes of gold stock. This year, these traditional jewellers have trimmed holdings considerably, in the backdrop of slackening demand for “new gold” (industry parlance for fresh cash purchases and not exchange of old gold). Zaveri Bazaar does about `170 crore worth of daily sales currently, as against Rs 225-250 crore worth of wedding season sales in previous years. Jain, whose family owns Umedmal Tilokchand Zaveri, one of the oldest shops in the bazaar, feels their patrons are “scared” to purchase new gold using banked funds, lest they come under the prying eyes of income tax officials.
Gold import and consumption data suggests traditional one-shop jewellers are fighting a losing battle. According to some industry-watchers, the shift (in buying preference) to organised players gathered pace after demonetisation last year. Unorganised jewellery shop owners are notorious for dubious sale methods. A good number are presumed to sell low quality (or smuggled) gold, issue fake bills and dodge taxes. Cautious buyers prefer to buy gold with proper bills.
“Buyers with clean money would not want to associate with small, unorganised players anymore,” says Surendra Mehta, national secretary, India Bullion and Jewellers Association. “They are moving to organised retailers now as small, one-shop jewellers are not even able to provide payment options such as digital pay-ins, RTGS or cheques.” Informed buyers insist on ‘hallmark’ a gold purity certification in accordance with Indian Standards specification.
Once the government imposes mandatory hallmarking of gold, over two lakh jewellers would be eradicated from the industry,” predicts Mehta. Today, consumers are also swayed by modern designs and light-weight options offered by organized retailers.

If an educator attends an out of state conference/ seminar, than it may include transportation, meals and registration fee. If a district or school organised this type of workshops than it may include cost of consultants, materials and so on.
- What is the content and is there a balance between research, theory and practice? The main content area-focused professional development training is help to achieve knowledge, practice and build content knowledge in subjects and also help to raise overall standards. Communication with suppliers is guarded with as little information as possible transmitted between the two parties in case one of them can turn it into an advantage at the expense of the other. The customer's best protection was thought to be the establishment of several alternative sources of supply for each item to ensure that competition kept suppliers 'on their toes'. Symptoms of this kind of adversarial relationship include multiple sourcing, a large supplier base, frequent switching from one supplier to another and buying solely on the price per piece. The result is usually mistrust, fear, dishonesty and frustration on both sides which prevents long-term quality improvement taking place. Clearly, supplier development requires a radically new form of customer-supplier relationship.
There are a few advantages of partnership.
First of all, easier sourcing, if we have already recognized a supplier that is the most efficient producer with the best quality and the lowest costs, why would we look anywhere else?
Also, Long term relationship. A long-term relationship eliminates the costs associated with finding and cultivating new sources. The time and energy that would have been spent can now be more profitably utilized.
Furthermore, Design capability. We expect the producers of our mission critical materials to be at the leading/emerging edge of technology. They, therefore, have design capability that we can capitalize on, not only in the development of future products, but also in the best way to incorporate their materials into our current products.
Besides, Technical support. We can expect technical support from our partners at all phases and levels of the process, from the design of the product through the manufacture of the product to support in the field. This includes assistance with any marketing efforts that require technical knowledge as well as any failure analysis that is required.
And Single- sourcing. It should not be longer necessary to split our requirements with multiple sources. Having a supplier that can deliver 100% of our requirement, 100% on time, with 100% quality, 100%of the time, is really all we need, which in addition to the saving of working with a single supplier also brings us the benefits of being able to leverage larger volumes.
But for the disadvantages, core strengths or competencies from both sides need to be understood and not be weakened, otherwise one sides will be eaten by the other side, merger and acquisition will be occur.
Also, the key technologies need to be kept and further developed, not shared. Otherwise the Lenovo and IBM case will be happened again.
Besides, due to the culture, politics, local law are different, but they are important. It may take time to coordinate.
Cultivating Supplier
Every now and again we have to grow our own suppliers. We need to start with a good foundation. Selecting a supplier from our preferred supplier list is a good first step. If we can find a supplier where our new items would be next logical step for the supplier, even better. It not, how easy would it be for the supplier to acquire and / or develop the necessary skills and equipment? We have a role to play here. We are going to grow this capability, which means we may have to lay out some capital, may have to provide some technical expertise, may have to do some training, and may have to baby-sit the development process. Helping our preferred suppliers is our contribution to the relationship.
Help cultivate the suppliers can help in some way.
First of all, improve supplier data quality, reduce duplicate entries, get the latest updates on addresses, contacts and other essential information to enrich your supplier data to help you work more effectively. Accurate information can provide more precise forecast. Provide near real-time access to information in a consistent manner throughout the organization.
Also, a very important matter, keep costs down. Better understand who you are working with to identify the right supplier mix to meet company goals and save money. By optimizing your supplier base, you can achieve greater savings, fewer disruptions and assure regulatory compliance.
But there is some disadvantage like if the supplies become stronger, their beginning power will be larger too, at that time they may not easy to communicate and control if we didn’t have a good relationship with them.
Also, when the supplier become stronger and have a large market share, monopoly may occur. And it may decrease the choice of another new supplier. It also is a disadvantage if cultivate the supplier too much.
Overhead cost Reduction
It is always important to remember that a portion of the price we pay is to cover the cost of overhead. And in many cases this far exceeds the raw cost of the product itself. Therefore, we feel very justified in reviewing those costs with the preferred suppliers. When we look at the whole picture with this fame of mind, we can begin to see other fertile areas for cost-related improvements that should show themselves by reducing the price we pay for materials. Overhead includes the cost of R&D, Finance, Design Engineering, Quality Assurance, Manufacturing, Engineering, Human Resources, Materials Management (which includes planning, purchasing, production control, inventory management, shipping and receiving), marketing, sales information technology, order entry, and manufacturing operations.
For the advantages, cost reductions in some of these areas not only reduce supplier’s overhead and make a more efficient producer but should also show up in our cost and in cycle time reductions.
But for the disadvantages, reduces overhead cost may reduce the flexibility and process improvement in a company. Restriction on innovation. Requirement of skilful personnel to set standards.
Also, there is a disadvantages of cost allocation, allocate more marketing efforts to free or low-cost channels may led to decrease the quality. And some of the company may control the labor cost in order to avoid paying overtime by fine-tuning scheduling or converting to part-time employees, this may also affect the loyalty and the quality of the goods dun to unskilled workers.
It would be unrealistic to assume that every kind of cost reduction is beneficial. As a general example, slashing transport budgets may result in holding higher levels of inventory. When inventory costs contribute more than transport to overall supply chain costs, as is often the case, this approach is clearly back to front. In fact, overall costs may be reduced, paradoxically, by increasing transport expenses instead.

Developing a sustainable competitive advantage in supply chain requires a new focus on creating value in a company. A supply chain needs to be viewed as a 'value chain,' in which all participants are truly integrated and share a common vision of goals, processes and information sharing.
First of all, partnerships can help increased market share, also help inventory reductions, improved the delivery service and quality.
Like the example The Philips Group, there partnership strategic is working together with the supplies towards a common goal. When partnership, both parties can gain more benefit through cooperation than by separately purchasing their own interests. And when the long-term partnership, they will continuously improve the product and clearly understand their responsibilities.
Also, partnership allows the free-flow of feedback and ideas. It can help to create a more effective supply chain that could have a positive impact on both costs and service. When there is information sharing, the product development will become easier to instigating new ordering processes and inventory control, that will be benefit both parties in the financial and operational area. Once the time and cost are lower, there are more resources to development new product.
Besides of the operation, partnership can also help to bring opportunities for long term contracts. As mall suppliers is trying to get a long-term contract that guarantees volume. This is also good for the suppliers because it can also allow them to schedule their production earlier. Long-term agreements can be used to detail expected redesign efforts that will provide cost reductions or quality improvements in the future. Suppliers are more willing to agree to these changes if they know they will have enough business in the future to warrant the investment. However, it’s hard to get a long-term contract, even 2-3 years contract are also harder. So, partnership can help both sides to guarantee the sales volume in order to occupy a certain amount of market share.
To conclude, Supplier development should lead to improvements in the total added value from the supplier in question in terms of product or service offering, business processes a performance, improvements in lead times and delivery for instance. There are many different supplier developments strategic that can help to improve the performance.
But every coin has two sides, each of the strategies have their advantages and advantages. It depends on how we use it and what situations we use it. We need to evaluate each of the strategies and select the ones we feel will have the best chance of success.
And one more thing, we may actually select more than one strategy, like all of the three above where we believe it is fitting. Because different strategies might work best in a different area.

Resentment of Rising Countries
The word 'resentment' comes from French and Nietzsche introduced it into German as a philosophical term. German Catholic philosopher Max Scheler (1874 ~ 1928) used the method of emotion phenomenology, focused on the spiritual temperament of people in the capitalist era, and discussed the phenomenon of resentment in the moral construction of the era, so as to push the study of resentment into a broad theoretical space. In the book Resentment, the author explores the emotion of 'resentment'. Regardless of the individual temperament and psychological structure of group members, the existence of these two conditions produces a tendency of group resentment.
Greenfield further argues that resentment creates a creative impulse to resist foreign values that can eventually lead to a 'revaluation', a shift in standards of value. The primacy of values has been devalued in favor of less important, external, or even negative aspects of the original standard. Out of pride, a society with a rich historical and cultural heritage cannot accept and accept foreign values. Resentment often leads to the selection and cultivation of elements of native traditions that are antithetical to foreign values, either to contend with them, or to dress them up as something similar to foreign values that is 'already there' at home, for psychological balance. Resentment breeds nationalistic pride and xenophobia, feeding the nascent national mood.
Ethnic identity is ancient in human history, but 'nation state' is a new concept. For a long time, the state was the private property of the ruler, and people generally recognized the royal family or religion rather than the nation state. In medieval Europe, it was not uncommon for a monarch to be king of two or more kingdoms at once, just as it is now possible for a person to be chairman of several companies at once.
According to Greenfield's research, England was perhaps the first modern nation-state in human history. It had been formed by the early 17th century, and its core concepts were citizenship, liberty, and popular sovereignty. Such national attitudes were later inherited by the United States of America, which formed in the late 18th and mid-19th centuries. Compared with England, other countries such as France, Russia and Germany developed their national identity relatively late: France in the 18th century, Russia in the second half of the 18th century, and Germany in the late 18th and early 19th centuries. Compared with the British and American civic nationalism, the Russian and German nationalism can be called ethnic nationalism, and France is in between. In the formation of the modern nation-state, the dominant group was the social, political, and cultural elite (the key group in England, France, and Russia was the aristocracy, and the key group in Germany was the middle-class intellectuals).
Firstly, France was the first nation to hold a resentment. For centuries before the 18th century, France was the hegemon of Europe and even the protector of Catholicism. But after the death of Louis xiv, France had clearly lost its dominant position in Europe. England suddenly rose to the center. When national ideas were introduced from England in the first half of the 18th century, the French found that the English outshone them in every respect, although France still had a proud list of outstanding scientists, philosophers and writers. France's national patriotism was marked by a strong demand to restore the dominance it had lost to England and win back the glory of France. There are two possible ways to ensure France's pre-eminence and glory: liberal reforms that turn France into a country like England; Or weaken England as a rival. Enlightenment thinkers such as Voltaire and Montesquieu espoused the first approach, without resentment, believing that France was capable of carrying out what it had learned from England and thus could easily outdo its role models and competitors. But the task proved harder and took longer than expected, and pro-British sentiment gave way to anti-British sentiment. The intellectual elite of the French aristocracy was convinced that France was basically comparable to England, but in reality, the former was second to the latter.
England's primacy hurt their national pride and bred resentment of the west. Under the control of resentment, the intellectual elite of the French aristocracy, on the one hand, stole the values of the English, on the other hand, they re-evaluated them. For example, the concepts of democracy, liberty, and equality were given a completely different, and sometimes quite opposite, meaning than in England. They replaced equality with 'uniformity', freedom with 'sovereignty' and 'common will', and individual with 'collectivity', paving the way for absolutism and equalitarianism. Resentment of the English has led the French to an instinctive hatred of money, business and capitalism. In the eyes of the French, England is a greedy people, a country with 'only a feeling for metals' and a capitalist society. A capitalist society, a people of injustice, greed, corruption, depravity, dominated by commercial interests, is not fit for France to emulate. In foreign relations, resentment often made the French aim not to make France strong, but to weaken England at all costs. For the French, the bad news for the British is their own good news. Support for the American war of independence among the French elite and the French public was driven in part by a love of freedom and affection for the American people, but more by hatred of the British. The separation of the United States from British rule was the most hated thing in France. But what Louis xv did not expect was that the financial difficulties caused by the war would become the trigger for the French revolution and lead to his death on the guillotine.
Secondly, the resentment affects the German nation even more. The German national consciousness emerged in the late 18th century, and it has taken root after the Napoleonic Wars. Therefore, France is the ultimate reason for the emergence of German nationalism. But at this time, Britain had ruled the world after the Industrial Revolution, and Germany was not a unified country. Not only was it not as good as Britain, but it was also even worse than France. This makes it difficult for the Germans to release. Although middle-class intellectuals learned the enlightenment of reason, political freedom, and individual equality from the 'advanced' Western nations (England and France), and some reformers even regarded France as an example to follow, their national self-esteem was hurt. By the beginning of the 19th century, as the national consciousness increased, the resentment of the Germans became stronger and stronger, and the 'Western World' (Britain and France) became the embodiment of evil, a negative example. The 'Abominable French Nation' is considered to be the 'natural and traditional' enemy of the Germans and a 'dirty, shameless, undisciplined race'; the English are generally no better than the French, and their freedoms are nothing value. As the West is increasingly equated with capitalism, German resentment toward the West (UK, US, and France) becomes resentment against capitalism. Next, Russia as a nation is also shaped by resentment of the west. The westernization of Russia was initiated and promoted by Peter the Great. At first, the Russians were envious of the west, seeing it as an absolute, unquestioned model, and optimistic that if they copied the west, they would be on an equal footing with it and then surpass it. For every shortcoming of the west, it has its own advantages; The west has shown its virtues, and it has them in reality. The value of 'reason' is preserved, but becomes similar to Hegel's zeitgeist and national spirit, rejecting its implicit individual creativity and autonomy. They recognize 'freedom' and 'equality' as virtues but refuse to recognize western institutions as their true embodiment because freedom and equality in the west are not true freedom and equality. True freedom is freedom of the heart, not freedom of the law. Westerners have the right to freedom but live in real slavery; Russians have no legal freedom, but they live in real freedom. The Russian elite invented the 'people', defined the 'people' as an ethnic group, the national spirit of Russia exists in the 'people', but only the educated elite can reveal it.
The elite, as the 'special people' (the vanguard), know what the 'people' want, and the masses themselves do not, so the former naturally have the right to command the latter. Therefore, the Russian nationalism formed under the domination of resentment is racist, collectivist, and authoritarian. This may help explain why Russia became the first socialist country. Regarding Elizabeth Bennet, she was the main character of Pride and Prejudice, being in my opinion, the bravest one. She did not mind what to say in order to express herself no matter what. She and her sisters were under the pressure of her mother in terms of seeking a husband. Elizabeth understood all the stress and difficulty of her family if she did not find a husband… but despite all the facts, she disagreed to go against her feelings, she wanted to be happy after all. When Elizabeth did not want to get married with Mr. Collins, her mother was so angry and disappointed in her, due to the fact that being single was a totally social disapproval. General Dress Code

For instance, Buddhist monks and Hindu pandits wear saffron-colored clothes while Christian Cardinals wear red robes etc. While these colors are forbidden, clothes with patterns of such colors are allowed; only solid-colored are prohibited. The colors recommended for all Muslims are white, black and green (Al-Munajjid, 1998).
Fabrics such as pure silk, velvet (Muslim, 5129), and such are prohibited for men;

Jabir reported Allah's Messenger ? as saying: When the thong of the shoe of any one of you is cut off. he should not walk with one sandal until he has got the thong repaired, and he should not walk with one shoe and he should not eat with his left hand and should not wrap his cloth round his knees or wrap himself completely leaving no room for the arms [to draw out]. (Muslim, 5235).
Islam discourages pride and therefore prohibits one to trail garments out of pride, “Ibn 'Umar reported Allah's Messenger ? having said: Allah will not look upon him who trails his garment out of pride.” (Muslim, 5191). This relates to young men nowadays wearing their trousers low, dragging around them on ground. This represent pride and gives a messy outlook of the individual.
For Sports
Sports clothing should not have impermissible colors. Having said that, it can have traces of those colors (Al-Munajjid, 1998). As for symbols and signs on the clothing, it is permissible to wear such clothes. However, if those symbols represent a religious group or allegiance or reverence to certain individuals, or represent hatred towards Islam, it is forbidden to wear them (Question 105416, 2009).
Usually men wear shorts and such while playing sports that tend to be too tight and short, compromising modesty. Sports clothing should be loose enough to not reveal one’s shape and form. Therefore, such clothing is impermissible to wear. “With regard to playing sport wearing only shorts which show the thighs or most of them, this is not permissible. The correct view is that young men must cover their thighs…” (Fatwa Of Shaykh Ibn ‘Uthaymeen From Fataawa Islamiyyah, 2003).
Clothing material should not be thin to the degree of becoming transparent. This is because it would not properly conceal one’s private parts and thus, compromise one’s modesty. For Prayer
One should take the utmost care when preparing for prayers, and that care should extend to one’s clothes as well. One must wear clean clothes, free from impurities. Hence, men whose clothes are dirty and foul-smelling from working and such should be careful and wear clean ones.
Apparel for prayer should not be tight, restricting movements during prayer. Tight clothes also contradict general Islamic etiquettes for dressing oneself and therefore should be avoided even in prayers. In other words, clothing for prayers should be loose and modest, concealing awrah.
Clothing for prayer should not be transparent because one of the prerequisites for valid salah is covering private parts. Wearing garments distracting one from full concentration during prayers should be avoided. Having said that, one should wear clothes pleasing to Allah because prayer is when we present ourselves to our Lord.
Conclusion
Islam has instructions and guidelines for everything, and there are sound reasons behind them; some we know and some we do not but, we trust Allah and His Messenger’s ? wisdom. We should take care of our clothing, whether male or female, because Islam has stressed it. Our apparel is a part of our outlook and appearance. One of the reasons Islam emphasizes instructions about clothing is because as Muslims, people judge us and our religion by how we look and dress ourselves.

Pride Can be the Sin of the Lord
It is a common idea belief that just because someone is perceived as a good Christian, they are incapable of sin. In reality, there is a bit of evilness in all good people. The same can be said for people seem to be the embodiment of evil; even ill-natured people can have a part of their soul that is the light, the good in all of us. In Flannery O’Connor’s “Revelation”, Ruby Turpin’s encounters with other females in the story show her true colors of judgement and pride. Mrs. Turpin’s interactions with the “pleasant woman” in the waiting room show that she considers herself to be God’s gift to Earth. She thinks that she is so perfect that everyone compared to her will always fall beneath her.
The only woman who Mrs. Turpin does not consider beneath her, but rather an equal, is this pleasant woman in the waiting room at the doctor’s office. She considers the pleasant woman as one of her own kind, she then sorts the rest of the room into categories that are beneath her. Ruby Turpin’s obsession with status and judgment of others is possibly the reason why she could be considered a “wart hog from Hell” (507). Her status levels range from “colored people” (491) at the bottom, to “people with a lot of money and much bigger houses and much more land” (491) at the very top of the ladder. The weirdest thing is that she will not place herself in the upmost level but she does consider herself quite close to there, “for some people with a lot money were common and ought to be below she and Claud” (491). The way Mrs. Turpin judges others and places them under where she places herself shows how much power she thinks she has and the amount of pride she has in her own self and social status. Pride, pride is one of the seven deadly sins and without a doubt the most common one for Mrs. Ruby Turpin. The above average ego of Mrs. Turpin can be seen through her interactions with the “white trash” woman, during which she uses her power to discriminate against others inside of her own mind because she thinks she is of higher importance than the rest of the world.
In regard to the white trash woman, Mrs. Turpin has many moments in which she talks with God, categorizing and ranking, in her opinion, the worst of the worst: “white-trash or a nigger or ugly” (492). This is most likely a way for her to boast how much better she is and boost her ego even more than it is already. Whenever the white trash woman speaks, Mrs. Turpin responds in her head with a hateful and rude comment. For instance, when the white trash lady tells how she bought a new piece of jewelry with the green stamps, Mrs. Turpin thought silently, “Ought to have got you a wash rag and some soap” (492). This happens many times, Mr. Turpin will say these rude and snarky comments in the safety of her own mind instead of owning up to what she thinks. Mrs. Turpin likes to hide behind the mask of a good faithful Christian, but under the mask she is nothing but a disrespectful seemingly evil being, and that is the worst part. It seems that throughout the story, Mrs. Turpin doesn’t notice that her initial reaction to others is horrid and sinful, but she still considers herself to be holier-than-thou.
The fact that Ruby likes to pretend to be Christian is ultimately what sets off Mary Grace’s rampage, and what presents the “old wart hog” comment. When Mrs. Turpin shouts, “Oh thank you Jesus, Jesus, thank you!”, Mary Grace throws the book at her head out of anger from her false Christian lifestyle. When Turpin does not confess to her faults when given the opportunity, her sins become exponentially worse. Her worst sin would be the difference in her inner thoughts and what she actually says and believes. By including the internal dialogue of Mrs. Turpin while she converses with the day laborer women, O’Connor puts the two-facedness of Ruby Turpin’s character on display for all to see. On one hand, she realizes how useless the opinions of the African woman are, but on the other, she continues to complain to them, almost as if she still wants praise and validation from these women.
Mrs. Turpin tells her story to the woman she has no respect for using a tone that one would talk to a close friend; however, in her head she thinks to herself, Idiots! She could never say anything educated to these women. Earlier in the story, Mrs. Turpin ensures the pleasant woman of how well she treats the workers on her farm. During that conversation, Mrs. Turpin says, “I sure am tired of buttering up niggers, but you got to love em if you want em to work for you” (494). Turpin seems to take good care of the workers and respect by bringing them water, providing them work, and talking with them, but these actions are all cancelled out by her inner monologue.
Mrs. Turpin’s treatment of the laborer women shows that she knows and does the right thing for the workers, but she does not love doing it. Actually, she says in her mind that she resents having to “butter up” or tolerate the African-American women. Just like in the rest of the story, Mrs. Turpin’s Spiteful and indictive interior is shadowed or covered by the nice and respectable air she puts on. In the end, Mrs. Turpin has an epiphany in the form of a vision. In her vision, “they were marching behind the others with great dignity, accountable as they had always been for good order and common sense and respectable behavior. They alone were on key. Yet she could see by their shocked and altered faces that even their virtues were being burned away” (508). This revelation that being arrogant and having this holier-than-thou attitude is the she can be a good Christian and an “old wart hog from Hell,” at the same time. Breach of Confidentiality Agreement
Introduction
Jennifer Lawson, the defendant, a former Junior Executive Secretary in the Research and Development department for Greene’s Jewelry Wholesale (the company) located in Derry New Hampshire. Mrs. Lawson is being accused of stealing Greene’s trade secret as well as breaching the confidentiality agreement. The defendant has countersued claiming wrongful termination.
Recently, the company started downsizing and eliminated some positions, this led to the defendant being laid off as she had just informed the company of her pregnancy. During her departure, the defendant is said to have taken documents outlining the company’s Ever-Gold trade secret. Immediately after, the defendant accepted a position with Greene’s competitors, Howell Jewelry World. The defendant worked for Howell for a small amount of time before being fired. The defendant now works for Triumph Jewels which is another competitor of the company.
Although the defendant did not sign any non-compete agreements, she did sign a confidentiality agreement where she could not disclose any information learnt about the company. The defendant has a high-risk pregnancy and claims this is the reason for her termination. The defendant received high scores on her yearly evaluations and the only thing that was against her was her habitual tardiness to work. The defendant did not file suit until after the company sued her first for leaking trade secrets.
Facts and Laws
Unlawful Termination
The defendant is claiming that she was unlawfully terminated due to her high-risk pregnancy. For the past three years she has gotten great annual reviews and besides the fact that she was habitually late, there was no other valid reason. Even more troublesome than that is that she gets laid off right after letting the company know of her high-risk pregnancy. This left the defendant unemployed and scared.

The defendant signed a confidentiality agreement upon accepting the position as a Junior Executive Secretary. This means that any information learnt while at the job or about the company must be maintained in secret. Even after leaving the company, any information must be kept a secret. In Mrs. Lawson’s case, it was the company’s trade secret, the process to create Ever-Gold.
While the defendant did not sign a non-compete agreement, she was expected to maintain secrecy about the process of Ever-Gold. It is believed that the defendant used that knowledge to obtain employment with Howell and even gave them the trade secret. Furthermore, it is assumed that after being fired from Howell, she used that same knowledge as well as any information from Howell to find employment with Triumph.
By breaching her confidentiality agreement, the defendant has the potential for the following violations of contract laws:
- Breach of Contract: A breach of contract is a failure, without legal excuse, to perform any promise that forms all or part of the contract (Breach of Contract, n.d.).
- Title 18 U.S.C. § 1832: It is an act that criminalized the theft of trade secrets for commercial or economic purposes.
- New Hampshire Uniform Trade Secret Act: It criminalizes the theft of trade secrets.
Precedent
While researching I encountered similar cases that could be used for reference like Coca Cola v Williams (Jarvis, 2008). Ms. Williams, a former employee of Coca Cola, conspired with one other to steal the trade secret and sell it to Pepsi through a third party. Ms. Williams snuck files and even a product sample out of her workplace. This case ruled in favor of Coca Cola because of the laws presented.
Similarly, Boston Scientific Corp. v. Lee had a lawsuit for breach of confidentiality contract. Dongchul Lee worked as a senior biomedical systems engineer for Boston Scientific and subsequently left to work for Nevro Corporation, a direct competitor in the spinal cord stimulation device market. Boston Scientific sued Lee, but not Nevro, alleging that Lee’s breached both his confidentiality agreement and brought work product and trade secrets to Nevro. Even though the court rejected the claim, we can learn what caused the claims to be rejected and avoid the same mistakes (Wurst, 2014).
Carbonara vs. Bank of New York Mellon Corporation has the same legal precedent. The bank didn’t lay her off because she was pregnant, she was terminated due to restructuring (Supreme Court, 2014). The court denied her claim due to these facts.
Facts to be Determined
The following facts need to be determined in order to better aid The Company in obtaining a favorable outcome for each of the legal issues presented.
- Proof of the confidentiality agreement signed by The Defendant. This will support the argument against Ms. Lawson if she violated the agreement.
- Did Jennifer Lawson’s belongings get searched upon departure If so, someone may be able to confirm the documents with trade secret information.
- Get a subpoena for the executive who offered Ms. Lawson the employment. This will help determine if she signed paperwork to offer the trade secret.

Abstract
Ethical Dimensions in Health Professions along with other related peer-reviewed articles thoroughly describe how to apply ethical principles, decision-making step-by-step, and clinical reasoning. A common ethical dilemma in health care involves confidentiality. Research shows that confidentiality is often breached amongst all healthcare members. If this information gets into the wrong hands, it can damage the patients’ reputation and cause extreme humiliation. Often times ethical principals are violated when there is a dilemma. Nonmaleficence and fidelity are both violated when there is a breach of confidentiality. Minimally patients deserve to be respected. Protecting and keeping their information private is a way to show respect.
The Hippocratic Oath
“And whatsoever I shall see or hear in the course of my profession, as well as outside my profession...if it be what should not be published abroad, I will never divulge, holding such things to be holy secrets.” (
This oath refers to confidentiality; one of the most basic principles in health care practice. Confidentiality is the practice of keeping harmful, shameful, or embarrassing patient information within proper bounds, the right to privacy gives legal standing to this ethical principle.
Medicine today is practiced by healthcare teams formed not only by physicians, residents, and nursing staff, but also by nursing assistants, orderlies, administrative personnel, and even students. Many people access patients’ charts routinely as a part of their jobs to obtain pertinent information to properly treat and care for them (Beltran-Aroca, Girela-Lopez, Collazo-Chao, Montero-Pérez-Barquero & Muñoz-Villanueva, 2016). This makes it increasingly important to protect patients’ confidentiality. The duty to ensure discretion and confidentiality in the medical profession is morally justified based on the rights arising from relationships, and medical practice involves trust relationships with both patients and society.
Confidentiality can be breached without realizing it, so it is important to watch what is said, where it is said, and to whom. On a daily basis, I hear chatter amongst medical staff about other patients’ conditions and behaviors. A lot of the conversations are in places that can be overheard by other patients, staff, and visitors. It boils down to carelessness, indiscretion, and sometimes even malice. Because this is so common, who do you call out or report? What can be done on a grand scale? I cannot imagine speaking to the head attending doctor about breaching a patient’s confidentiality. Nurses must be confident enough to speak up no matter the situation or status of the professional they are addressing because we are the patient’s advocates.
My Dilemma
An ethical dilemma occurs when a moral agent is faced with two or more conflicting courses of action but only one can be chosen as the agent who attempts to bring about an outcome consistent with the professional goal of a caring response. An ethical dilemma that occurred on my unit involved a breach of confidentiality that led to discrimination and refusal to care for a patient. A nurse was giving report to another nurse on a patient who was positive for HIV. Report took place at the nurses’ station where most staff and sometimes families migrate. Anyone within 10 feet away could hear. One of the personal care assistants (PCA) overheard the report and began their own opinionated side conversation about this patient. For a chunk of the shift, this patient was singled out. Whenever someone would enter the patient’s room to help with repositioning or transporting the patient; the PCAs would warn them about the patient’s HIV status. This information was not necessary to be revealed to others. There are two general exceptions where it is necessary to question whether to maintain confidentiality: when the safety of others or public health is threatened. HIV is not contagious; so, the safety of the staff assisting this patient is not threatened. Using appropriate PPE for blood draws and peri care is standard.
Ethical Principles
Some health information is subjective impressions about the patient and their lifestyle choices and habits. Improper disclosure of highly sensitive information could damage a patients’ reputation or result in loss of opportunities. In this case, the principle that was violated is nonmaleficence; to avoid causing harm to the patient. Constantly exposing the patient’s medical history to staff who were not involved in the patient’s care could lead to personal humiliation and embarrassment. Fidelity was also violated. Fidelity is being faithful to one’s commitments to the patient and meeting their reasonable expectations. Patients minimally deserve to be respected and to keep their personal information private. The PCA also refused to draw blood on this patient. It is unethical for the PCA to refuse to assist a patient because of their health status.
There are no special precautionary measures that need to be followed when treating a patient with HIV because it is transmitted via bodily fluids. Only standard precautions need to be used; no different than most patients. I would address the first step of the decision-making process by gathering as much information as possible. The PCA’s actions were very discriminatory. Because of the stigma associated with this disease, I would assess her knowledge of the disease. What is HIV? How is it spread? What are their feelings about the disease? Would they treat the patient any different if it was a family member? How much do they know about this patient? Do they understand standard precautions and what PPE is required to draw blood for an HIV patient?
This brings me to Step 2 of the decision-making process; identifying the type of ethical problem. This situation caused moral distress. Other staff simply ignored the inappropriate behavior to avoid conflict with this senior staff worker. I knew I had to be the patient’s voice no matter what.
In step 3, the ethics approach that addressed the dilemma the best is deontology; which focuses on duty. Everyone who witnessed this behavior was moral agents that did not act on behalf of the patient. As a moral agent, it was my duty to protect this patient from humiliation.
Step 4 is exploring practical alternatives. I debated with myself if I should talk to the PCA first, contact the ethics committee, go directly to my manager, or do nothing at all. The ethics committee would entail involving more people that could cause further embarrassment and humiliation to the patient especially since she was not aware of any of it. Doing nothing would not benefit anyone going forward.
Step 5 is completing the action. I decided to talk with her first to gauge her understanding of the situation. I explained to the PCA why her actions and refusal to do her job is unethical. I also gave her the opportunity to reflect on her decision in hopes she would ultimately reconsider and do the right thing. She would not budge. I completed the blood draw myself. Although with hesitance, I reported this PCA to my manager. My manager, also a moral agent, was very upset by what she heard. She sent out a nonspecific email about HIPAA policies along with a reiteration of bedside report for confidentiality reasons. Step 6 is evaluating the process and outcome.
Unfortunately, during the meeting, the PCA did not own up to anything; giving an unrelated excuse of why she did not draw the patient’s blood. My goal was not for her to be punished but to be made aware of her actions and how they could affect patient-staff relationships. The awareness of her actions led to a discussion amongst the other PCAs and nurses on the unit. The potential for repercussion changed how report was delivered and how patients were cared for. Nurses are now more discreet or give report at the bedside so that other people cannot overhear private information. All other staff is likely to treat all patients the same since their diagnosis is unknown. If I had not reported her, many others would think their job duties were optional and believe it is okay to violate HIPAA and discriminate against others. As medical professionals, we have a higher moral standard to uphold.
Clinical Reasoning
Narrative reasoning applies to my ethical dilemma. It is a framework for understanding the patient’s “life story” or illness experience. After building a rapport, I would try to understand the patient’s perception and feelings surrounding her diagnosis and all things leading up to it. If this is not a comfortable topic, I would focus on her background and culture and how these things may have contributed to any illnesses she has.
Ethical reasoning applies to my ethical dilemma as well. It is a mode of reasoning used to recognize, analyze, and clarify ethical problems that arise. The focus is not on what could be done for the patient, rather on what should be done. I struggled with deciding what the right thing to do was. Talking to the PCA gave her a fair chance to express her behaviors and correct them. Getting management involved was the best course of action afterward in dealing with a breach of confidentiality and discrimination. This may have prevented similar situations from occurring again.
Everyone is Involved
In an article about confidentiality breaches in clinical practice, a study was done involving 5th and 6th-year medical students at the University of Cordoba. This study involved direct observation in real situations where a breach of confidentiality occurred. They measured the type, severity, frequency of the occurrence, medical department, and the type of medical professional involved. The goal was to make health professionals and hospital management aware in order to implement measures to prevent future incidents. This study proved that although most incidents were committed unintentionally, every single healthcare personnel was in some way involved in confidentiality breaches.
Ethical Workplace Issues
A common legal definition of a whistleblower is someone, typically an employee, who discloses information, either internally (to managers, organizational hotlines, etc.) or externally (to lawmakers, regulators, the media, watchdog organizations, etc.), that he or she reasonably believes evidence: a violation of law, rule or regulation; a gross waste of funds; abuse of authority; o a substantial and specific danger to public health or safety.
Diverting narcotics is an ethical dilemma that is a common occurrence amongst health care staff. It can be easily missed due to improper wasting. In the hospital, the Pyxis will not allow you to waste a narcotic without getting a witness. Both nurses would review the amount to be wasted and would waste it right in the medicine room in a specific bin. I noticed some nurses will sign as a witness but would not stay to watch the other nurse physically waste the narcotic. That leaves opportunity for diversion. Also, it is easy to divert narcotics for patients on the CIWA scale because the amount of Ativan given is based on their behaviors and observed data. 2 mg can be taken out but only 1 mg could be given.
Diverting narcotics affects the safety and quality of care for patients. Not only are they not receiving appropriate doses to be therapeutic, but they are being treated by nurses under the influence. This poses a safety issue. Impairment can lead to medication errors and unsafe patient handling. Nonmaleficence was violated because the behavior of the nurses can potentially cause harm to patients. Patients consistently reporting unrelieved pain after receiving narcotics can potentially be an indication. Witness unusual behavior from the nurse involved can also be an indication of narcotic diversion. This unusual behavior can lead to medical errors and unsafe medical practice. One action as a moral agent in this scenario is to confront or report any unusual behavior to the charge nurse or nursing supervisor. Witnessing all wasted narcotics from start to finish will prevent many cases of diversion.
Lateral violence is another common ethical dilemma The term lateral violence refers to a pattern of workplace conflict in which confrontational behavior is targeted at 1 person by another employed at the same level of responsibility. The behavior is typically demeaning, humiliating, and diminishes the victim’s stature, resulting in social exclusion. Nearly all nurses experience lateral violence in their careers. One study of lateral violence found 97% of nurses surveyed reported lateral violence in their health care workplace as a common occurrence. The highest frequency of complaints focused on patterns of verbal abuse.
Lateral violence makes the work environment very uncomfortable and causes unnecessary emotional stress on those involved. Ultimately this can affect the quality of care provided to the patients. If the nurse dreads going to work due to bullying and conflict with fellow staff members, it can come across to patients. The ethical principle that is violated in this dilemma is fidelity. There is a reasonable expectation that basic respect will be shown to anyone, anywhere. Anyone who victimizes and singles out an individual is abuse. I was involved in lateral violence. I was new on the unit and a very outspoken staff worker mentioned I was pregnant to everyone at the nursing station. She had heard the news from my preceptor. I was embarrassed and upset because it was a secret I was not ready to share. I expressed to her that it was not her personal business to share. Instead of expressing remorse she turned most of everyone against me. I would hear her talking badly about me to other staff members and constantly belittling me due to my lack of experience. I did not report her to HR because I was afraid of retaliation. If I witness workplace bullying against someone else, I would report them to management or HR.
Keeping a patient’s private information confidential is the duty of all health care professionals. If there is a breach of confidentiality witnessed, it should always be addressed. Because it occurs so often; mostly without realizing, it is important to make each other accountable for how our comments can affect our patients. Limit discussions regarding patients to those who are directly involved in the patients care. Giving report in secluded areas where information cannot be overheard is ideal. Keeping patients’ information confidential demonstrates respectful and helps build upon trusting relationships amongst patient and staff.

Patient Confidentiality Must be Protected
We all have gone to our doctors and nurses for an appointment and we all have to trust that them with our information. We have to trust that they respect our right to having patient confidentiality. Patient confidentiality is when a person’s personal and medical information is given to a health care provider, that information will not be disclosed to others unless they are given permission from the owner. With that being said, should the confidentiality of a 13 year old girl be broken because she’s younger and her mother ask for it.
According to the AAFP organization, it states that “A confidential relationship between physician and patient is essential for the free flow of information necessary for sound medical care. Only in a very setting of trust will a patient share the non-public feelings and private history that modify the doctor to understand absolutely, to diagnose logically, and to treat properly. The yank Academy of Family Physicians (AAFP) supports full access by physicians to all or any electronic health data among the context of the medical home.”
I believe that patient’s confidentiality should be protected. In the past, the privileged nature of communications between physician and patient has been a safeguard for the patient’s personal privacy and constitutional rights. Data sharing is a lot of work, significantly across state lines given differing state patient privacy/confidentiality necessities. The AAFP believes that state and federal legislators and jurists ought to request a bigger degree of standardization by recognizing the subsequent principles relating to the privacy of medical information:

According to the Perspective in Clinical Research website it states that “Information of a patient should be released to others only with the patient's permission or allowed by law. When a patient is unable to do so because of age, mental incapacity the decisions about information sharing should be made by the legal representative or legal guardian of the patient. Information shared as a result of clinical interaction is considered confidential and must be protected.” With that being said, no matter what the duty of waring is the nurse lawfully has no right to tell the significant other anything.
Another example of the ethical dilemma of patient confidentiality is security breaches. There security issues breaks patient privacy once confidential health data are formed on the market to other people while not the patient’s conformation was not given. There was another two recent incidents at Howard University Hospital According to Washington it showed that inadequate knowledge security will have an effect on an oversized range of individuals. On May 14, 2013, federal prosecutors charged one of the nurse in the hospital's medical technicians with violating the insurance movability and responsibility Act (HIPAA). In the article it states that “Prosecutors said that over a 17-month period, Laurie Napper used her position at the hospital as a way to gain access to the patients' names, addresses and Medicare numbers in order to sell their information for money”. There was a plea hearing for what she did on June 12, 2013 in which she was found guilty and sentenced for 6 months and fined a total of $2,100. A few week later the hospital let every single one of the patient’s whose information was sold what had happen and ways for them to fix.

Please Don’t Tell: a Case about HIV and Confidentiality
The article main conclusion, is it the physician duty to warn third party which is the patient sister (Consuela) about his patient (Carlos) HIV positive status since she will be taking care of his wounds at home for her own safety. The article stated reasons to support the main idea, Fleck assumes no need to breach confidentiality if there is a home health nurse, but should be aware of universal precaution. The chance of her getting HIV is remote at best. Angell argues Consuela should be informed about Carlos HIV positive, the doctor should breach confidentiality. Since she's being generous to Carlos by providing care, in return Carlos must tell his sister he is HIV positive or ask the doctor to tell her or he can decide not to tell her and do without her nursing care.
Fleck argues that breaching confidentiality would be unjustified. He provided reasons that should be met before a physician breaches confidentiality that can be justified. He stated, when there is an imminent threat of serious, irreversible harm; there is no alternative way to averting threat; and the harm that would thereby be avoided is proportionate to the harm associated with breaching confidentiality. All the reasons Fleck stated above, none of them are imminent threat to Consuela and the risk of transmission during wound care is very low.
Breaching confidentiality is considered against HIPAA, if the patients informs physician not to warn the third party. Carlos had reasons why his physician should not tell his sister about his HIV positive because she may decline or be afraid to provide him care or other family members might know about his sexual orientation and health status. I think that the sister should be informed of the importance of and thoroughly educated in the practice of universal precaution. I also believe Carlos should be educated in universal precautions and should monitor his sister to insure that she follows the precautions for her safety since he chooses not to tell her about his HIV positive status.
I believe Angell argument is the weakest because he states the physician should breach patient’s confidentiality. He believes Carlos should either tell his sister about his condition or forfeit her nursing care. I do not agree with Angell because it is not important for the doctor to breach confidentiality in this situation since the risk of infection is so low, if Consuela follows the universal precautions correctly, compared to the risk of Carlos informing Consuela about his HIV positive status. He may harm himself or commit suicide if he realizes after informing his sister about his sexual orientation and health status. She starts treats him differently as an outcast since it is against their culture. I believe a time will come when Carlos is ready to inform his family. For example: As a nurse aide at times I draw blood from patients who are diagnosis with sexually Transmitted Disease without my knowledge. Some nurses choose to inform me for my safety other do not. But since I use universal precaution I am always safe.

Explain how Coaches should Contract and Manage Confidentiality to Coach Ethically
At the start of any formal work relationship it is important to agree a contract, an agreement about how you are going to work, the terms and conditions of the relationship. Coaching is no exception and in a similar way to other work arrangements there is usually a need to agree a formal contract and also an informal contact.
The formal contract can either be a two-way agreement, between the coach and the individual, or more usually, three way, between the coach, the coachee and the manager of the coachee. In some organisations the Human Resources Department has a role to play and is often responsible for the contract and can also be part of the formal contracting. This formal contract, which is usually a written document, spells out the commitments of the coach, the coachee and the manager in the coaching process.
The coach needs to spell out clearly for the coachee and the manager how they plan to work, the schedule of work eg 6 coaching sessions over a 6 month period each lasting one and a half hours. The cost needs to be transparent and agreed up front, both fees and expenses. This is usually a cost for the full schedule of coaching. Payment details, how the coach will be paid, in one lump sum at the beginning or monthly as the coaching progresses, need to be agreed. The coach needs to understand where the coaching meetings will take place. Often coaches like to be coached away from the work environment and in this case a budget for booking private rooms in a hotel or other venue need to be agreed. Often once the coaching is underway the coach and coachee can agree to meet in a more informal meeting place eg a café or hotel lounge area. Cancellation arrangements also need to be agreed. The coach will normally need to be flexible about changes to meeting times as the coachee will sometimes have last minute changes to schedule. However there need to be parameters agreed on these changes eg. changes within 24 hours of the meeting will mean that a missed meeting charge will be made.
Termination terms need also to be agreed. If for any reason the coachee or the manager wish to terminate the contract it is usual for 2 weeks written notice to be given.
The coaching contract also needs to cover the overall goals for the coaching process. The manager will have spoken to the coachee about coaching in advance, when they offered them the opportunity to have a coach, so this discussion should not be news to the coachee. It is important that the discussion about the goals of the coaching is discussed with the manager and the coachee so that there is no room for misunderstanding. The coach and the coachee will have met before the formal contracting meeting as they will have had a chemistry session to ensure that they can work together. The goals of the coaching will normally have been touched on at this point. This is therefore a formalisation and double check that everyone understands the goals.
Once the schedule of the coaching has been agreed the manger or sponsor needs to commit to releasing the coachee for the agreed amount of time. Coaching is often most advantageous if done either at the beginning of the day before the demands of the day take over or at the end of the day when the days work is behind them and the coachee can focus purely on themselves and the coaching. Half an hour before the coaching session is scheduled should ideally be put aside for the coachee to reflect on what they want to cover and to have a chance to clear their mind so that they can be fully present and concentrate on the coaching. It is also a good idea for the coach to work to this discipline and set themselves half an hour before running a coaching session so they can also be clear on the purpose of the meeting and what they intend to cover or think through any techniques they plan to use.
The manager needs to commit to doing everything possible to allow the coaching to take place at the agreed times and not to pressurise the coachee to cancel if a last minute problem occurs.
The sponsor also needs to commit to being supportive of the plans that are agreed at coaching and give any help and support needed for them to be implemented. The manager has a duty of care to ensure that the coachee finds the experience as fulfilling and developmental as possible and that they feel able to implement changes agreed as a result of the coaching when back in the workplace.
The coachee may have some personal goals that they wish to make for the coaching sessions. They may be open to sharing these with the manager or sponsor in the contracting meeting or they may want to wait until they are alone with the coach to agree with them. In some instances, coaches want to use the coaching sessions to help build a better relationship with their manager and they may not wish to raise this in front of the manager. All coaches need to feel confident that they are working with their coach in a safe environment.
The coach needs to be clear in the formal meeting about the confidentiality and the non-judgemental nature of the sessions. They will commit to not disclose details of the discussions but only report on the achievement of goals if required. Often the coach is not required to report back to the manager throughout the coaching period, the manager gets their updates from the coachee and hopefully sees the benefits of the coaching through different behaviours displayed or a change in the ways of working that is aligned to the goals set. It can be agreed that the coach will meet with the manager and the coachee at the end of the coaching period and contract to review progress made and if there is any further requirement for coaching or any other developmental intervention.
It is crucial that both the coachee and the sponsor feel confident that the coaching will be done in an ethical way. If the coach is an external coach to the organisation they should be able to prove that all coaching records will be kept in a locked cupboard or in a passworded file. External coaches should have a privacy policy which they should provide for the coachee and the sponsor to read. They should also discuss their commitment to abide by a code of ethics which includes abiding by all employment legislation, avoiding unconscious bias, taking care not to discriminate, either directly or indirectly, in any way during the relationship. They need to make the client comfortable that they are familiar with employment legislation, the Equality Act and Privacy legislation. They need to be ble to convince the client that they will create a safe environment for the coaching.
Once the formal contract has been agreed and signed off the informal contract between the coach and the coachee can be agreed. This is held at the very beginning of the coaching process but can be revisited during the process if required on either side. Whereas the formal contract was about the ‘what’ of the relationship, this contract is about how they will work together. The acronym COACH is a useful way to remember what to cover in an informal contract.
Confidentiality – the coach is bound by coaching ethics to respect confidentiality. This means that everything discussed in the room is confidential and will not be repeated by either party outside the room unless specifically agreed to do so. Even if the sponsor asks the coach directly to discuss the coaching sessions they will not discuss unless agreed with the coachee.
Openness – the coachee must agree to be open in order to gain the most form the coaching and to see real progress towards the goals set.
Accountability – the coach will hold the coachee accountable for taking agreed action and will usually start each session with a review of what they agreed last time and catch up on progress made.
Challenge – the coachee needs to be willing to be supportively challenged. The coach is likely to ask questions that are outside the comfort zone of the individual and they need to be agreeable to this from the start if real progress is to be made.
Honesty – the coachee needs to be honest with themselves and the coach and bring their whole self to the coaching. This is a session just for them and they need to feel that they can give it their full attention for the duration of the session.
Sir John Whitmore touches on the importance of ‘conscious working agreements’ in his book, Coaching for Performance. ‘The conscious and deliberate design of a good working relationship at the start builds the respect, trust, and agreement that are essential to collaboration and high performance’ (p63)

Dentistry is the Career i Want to Pursue
The sky seemed a little too dark, so I changed it completely. I added in some reds and yellows and changed a dark gloomy day into bright and beautiful sunset. I then went in and added the small details to lighthouse and to the mountains in the background. The sky then seemed empty, so I added in some birds. Finally, the ten hours I spent working on this painting had paid off, it was finally complete.
To others, it may seem that the true joy in creating a piece of art is seeing the completed work, but for me, the true joy comes from the process. I love spending hours staring at my canvases, filling them up, adding in details, and then getting new ideas and starting over again. As tedious and frustrating as it may seem, it gives me pleasure in knowing that if I am patient and persistent, the end result will be something that both I and others can enjoy.
Dentistry, like painting, is a form of art that is very focused on the process rather than the outcome. My interest in dentistry first began when I shadowed my own dentist during my senior year of high school. I remember this one patient who came in for dentures and needed to have her lowers pulled. I watched as the doctor extracted about five teeth. Some came out easily, while others required work. Each tooth he pulled produced that same gratifying feeling that I had when I completed a part of my paintings. When he later finally placed her new dentures in her mouth, the smiles on both of their faces led me to believe that I had just witnessed an artist create his masterpiece. The precision and patience that each step required left me in awe and that was when I knew that dentistry was the career I wished to pursue.
My interest bloomed into passion, when during my freshman year of college, I went on a dental mission trip to Guatemala. This was also my first hands on experience with dentistry. Most of our patients were children, and because there were only three of us, we were working back to back from morning until evening with very little break. We would set up our trays and brush and floss their teeth and teach them along the way, and their bright smiles and big hugs made the long hours we put in worth it. The one defining moment during this trip was when we had a woman, Maria, come in with excruciating pain in her jaw. The pain left her unable to work and provide for her family.
She needed a root canal which was something that she could not afford, and we could not do with our limited equipment. In order to alleviate her pain, the doctor decided to extract her tooth instead. The roots of the tooth were so curved that the extraction ended up taking over an hour. We stood there with the doctor, assisting her as she tirelessly worked away with precision, never letting her frustration show. Being a part of this process, as well as the whole trip, gave me a deeper appreciation for not only the amount of work that can go into the smallest procedures, but also the amount of faith that the patient has within their dentist. These people trusted us to take care of their problems, and some like Maria literally put their entire futures within our hands.
I was able to take all that I learned from these experiences and apply it to my volunteer position at House of Hope. I volunteered at their food pantry, and although it may not seem like it, we played a big role in shaping people’s futures. As dentists do with their patients, I was able to create relationships with the people that came in. At first, I was just a pair of ears to listen to their stories, but eventually, I became a source of friendship and companionship. It was beautiful seeing and being a part of the process of them going from being helpless, to being able to completely support themselves. This experience also taught me about empathy and how powerful it truly is. My believing in these people and being there to support them, helped them to believe in themselves.
Through all of these experiences, I have learned that there is an entire process full of struggles and obstacles that one has to go through in order to create a masterpiece. Some like those at House of Hope struggle to make a life for themselves, while others, like the dentists I shadowed, go through struggles to give their patients the best care. I know that dentistry is not an easy career and that it will not only get harder as I move on, but I am determined and ready to work as hard as possible, and I hope that all I have achieved thus far is a testament to that.

Also discussed will be dentistry instruments that are used to examine and treat dental problems in animals.
In equines, the deciduous teeth formula is 3/3, 0/0, 3/3, 0/0. Adult dental formula of an equine is 3/3, 0-1/0-1, 3-4/3-4, 3/3. The equine canine teeth, or tushes, erupt between four to five years of age and are short and can be sharp. They are located in the diastema, or gap, between the incisor teeth and cheek teeth on both the upper and lower parts of the mouth. Canine teeth are usually only found in male horses; but there sometimes can be canine teeth found on small mares.
The wolf teeth are small teeth and are found in front of the first upper cheek teeth. They erupt between six to eighteen months of age varying in size and position. Very rarely they are found in front of the first lower cheek teeth. Not all equine have wolf teeth and not all equine have fully erupted wolf teeth on each side.
Cheek teeth are the teeth that do all the grinding of the grass they eat. They help aide in digestion of the grass. Equine have a total of twelve premolar and twelve molar teeth that are divided equally on the left and right side into an upper and lower level of six. Premolar teeth have a deciduous precursor and are shed as a cap. In a healthy equine the cheek teeth arcades are composed in a tight unit. These teeth each act individually as a grinding tooth.
Dental disease can be prevalent in equines. It may cause pain or irritation where the horse may chew their food for a little while then drop it. Equines may only eat on one side of their mouth or pack food in their cheeks. If an equine is salivating more than usual they may be doing that to soften the food to make it easier to chew. An equine may also act out negatively when instructed to do something due to the pain. Long term problems include weight loss from not eating enough feed.
Besides feeding issues, mouth odor can be another indicator of oncoming dental disease. It can be a sign that there is food stuck in the diastema which leads to the disease. Food can become packed in that space and eventually start to decay. When the food decays it affects the gums, the tooth and surrounding bone causing infection. While some diastema will correct itself as the horse ages, in the cases where it does not, cleaning and packing of the gaps may be needed.
Pulpar disease in equines is often unnoticed. Equines do not have a nervous supply to the pulp so do not feel pain if the pulp is infected. The pulps are visible on the surface of their teeth. Simple exam can indicate if the pulp is infected. The tooth can cause jaw and facial swellings that can be painful or warm. This can lead to malodourous discharge. The upper and back teeth have roots into the sinuses. Swelling may go unnoticed and burst into the sinuses. That then will cause malodorous nasal discharge.
Dental caries in the equine is the destruction of dental tissue by bacteria in the mouth. Caries are found on the side of the teeth, (peripheral caries) or on the grinding surface of the upper cheek teeth, in an area known as the infundibulum. Peripheral caries usually do not cause problems. However, infundibular caries are more disposed to tooth fractures because they weaken the tooth.
Periodontal disease is one of the most painful diseases that can occur in an equine’s mouth. It causes premature tooth loss in adult horses. It can affect incisors, canine and cheek teeth. Equine teeth continually erupt over its life span. Tissues and surrounding teeth undergo normal changes as eruption happens. The periodontium contains the gingiva, alveolar bone, periodontal ligament and the cementum of the tooth. When there is normal mastication the mouth of the horse is self-cleaning. Feed and calcui do not accumulate on or around the teeth. This is part of the normal defenses such as salvia, white-blood cells, antibodies, and good bacteria.
In the field, veterinarians use specialized instruments to conduct an oral exam. Some are hand-held and some are powered. The stethoscope is used to check heart, lungs, and trachea before the examination. This is to ensure the airways and heart rate are normal before sedation. When the horse is sedated, do not make a lot of noise as it may startle them. A steel bucket with rubber on the bottom is used to prevent the instruments from making noise while being carried in the bucket. In a separate bucket with water, a little detergent and soap is added and used to rinse the horse’s mouth with a dose syringe. It is sprayed into the mouth to rinse it before the oral exam. A full mouth speculum is fitted to the horse right into the mouth. It enables the veterinarian to put a hand in the horse’s mouth. A head rest can also be used to support the horse’s head. For light sources a head lamp is worn to light up the mouth allowing the exam and teeth work to be conducted hands free. It is important to ensure that the horse does not eat for ninety minutes after any work is done. A horse muzzle can be put on the horse to prevent them from eating or drinking water.
There are multiple types of hand held instruments for the examination and work on equine teeth. Dental mirrors are used to visualize fractures and fragments of teeth that may be invisible without it. Dental picks come in assorted sizes to check the teeth to see if there are holes or grades in the teeth. Extractors are used to pull out baby or cheek teeth. Cheek retractors are used to remove the cheek from the teeth to work more freely. Surgical sets, which contain elevators, are designed for removal of wolf teeth.
Power tools can also be used to assist. Carbine burs help smooth out sharp points of the horse’s teeth. Hearing guards and masks are used to protect the veterinarian from dental dust. It is not good to inhale the dust in everyday. A tank with high pressure can be used to rinse the horse’s mouth after procedures are done.
In an equine hospital, similar instruments are used as in the field: a speculum for the mouth, and a dental mirror for observation. However, there are other tools that a hospital has at its disposal. One is the periodontal probes. As with human dentistry, the probes help to examine areas of the teeth that appear abnormal or where pockets may be forming from oral decay. Water picks aid in the cleaning of diastemas. This helps a more thorough exam be done on the mouth. Molar pullers are used to pull loose teeth that may be causing the equine pain. Digital radiography is used in aid of visualization of teeth. This helps the veterinarian to see if there are any root abscesses, look at the tooth structure and surrounding bone.
Dental procedures are performed to create a good environment for chewing, nutrition and to prevent any future problems. Floating is done to removal sharpness on the upper and outer cheek teeth. Equilibration is removal of overgrown areas of cheek teeth. This allows all teeth to touch when and how they are supposed to. That is what helps makes mastication more efficient. Another procedure helps correct malocclusions which occur between the cheek teeth and incisor teeth. Malocclusion causes poor grasping or chewing of food and loss of teeth if not corrected. It can even lead to behavior problems and disrupt performance.
Bit seats are a very important part of dental care in the performing equine. They help to round and to angle the upper and lower first cheek teeth to prevent the bit contact with cheek teeth. Bit seats help to give the rider better control by eliminating the distraction when bit contacts with teeth. It also helps the bit seats from riding down the tongue.
Canine tooth reduction is the process where the teeth are rasped and contoured. It helps to shorten them. While some equines benefit from them, some do not. Each dentist should evaluate their patient to see if it would be a benefit. Transverse ridges are surfaces on teeth that are too tall or have a sharp point at the top of the tooth. If not taken care of they interfere with chewing and back-to- jaw motion. Wolf teeth are small teeth are found in front of the first upper cheek teeth. These are not functional for grinding and are usually removed. This prevents pain in the horse when it is ridden.
Post dental care involves observation of the equine. How is the equine when it is chewing its food? Is it keeping the feed in the mouth or is it dropping some of it still? Does the equine need to be confined or have a muzzle put on its mouth for ninety minutes? This will prevent the horse from eating until it is awake enough from the sedation. Does the equine need antibiotics or pain medication? Some procedures may be clearing up an infection, some are painful. Scheduling dental exams with the veterinarian are important while remaining diligent in the equine’s dental needs throughout its lifetime.
A rodent’s dental formula is Incisors 1/1, Canine 0/0, Premolars 0/1, Molars 3/3. Their teeth are known as heterodont dentition. Instead of canine teeth they have a long diastema between incisors and cheek teeth. There are two types of teeth that the rodent has: aradicular hypsodont and, in some species, brachydont cheek teeth. Upper and lower incisor teeth come to a chisel-like point. Enamel is on the front and lateral sides of the teeth only while cementum and dentin are on the palatal/lingual surface.
This explains why the incisors are more chisel like - the cementum and dentin wear a lot faster than enamel. Enamel of the incisors in rodents takes on a yellow-orange color. Hypsodont cheek teeth are often angled rather than flat occlusal table. The highest point of the chisel tip maxillary cheek teeth is on the buccal side angling towards the soft tissue of the hard palate. The bevel of the mandibular cheek teeth goes in the opposite direction. The chisel point of the mandibular cheek teeth is on the lingual side. The tooth’s occlusal table angles towards the soft tissue of the cheek. These wear patterns are due to the maxillary cheek teeth being spread wider apart at the midline than the mandibular teeth. This is termed anisognathic, or unequal jaw relationship, meaning the upper dental arch is slightly wider than the lower. Periodontal ligament of permanent aradicular hypsodont teeth may differ from brachydont teeth. This is due to the presence of the periodontal ligament between its tooth and bone attachments. The plexus may allow continually growing teeth to move upward as they grow.
Some dental issues seen in rodents can be a result from insufficient gnawing; as their teeth continuously grow this can lead to overgrowth of incisors. Incisor injury can lead to malocclusion. Rodents are also prone to cavities and periodontal disease. This is caused by the same bacteria that cause cavities in humans. Rodents can get this bacterium from kissing their owners and sharing food. Poor nutrition can lead to dental and bone abnormalities. Other problems that can occur are decreased appetite and weight loss. Saliva will build up on the inside of the lip, under the chin and inside of the front legs. The rodent may show reluctance to eat anything hard or anything at all. Their breath will stink. Lumps can appear under the cheek or eye or even the lower jaw. Increased tears can occur in one or both of the eyes. Chewing movements can be seen in the rodent’s mouth when it is not eating. If incisors are present and uneven most likely the cheek teeth are abnormal also. This can also to lead to trouble closing the mouth. Slobbers is a common dental disease that rodents get. That is where drool is evident and a constant wet chin. Abscesses can develop in the rodent. An abscess is a thick pocket of pus that is surrounded by a capsule of thick tissue. Rodents have thick creamy pus that does not drain well. The abscess is their body’s way in fighting off infection. Since the thick tissue prevents antibiotics from getting to the infection, surgical intervention is usually needed. If the abscess becomes large it can affect the teeth and the surrounding bone.
Instruments and equipment in dental care for rodents are varied. Towels can be useful to help restrain them. A laryngoscope with a small pediatric blade is used for endotracheal intubation to maintain the airway and to administer anesthesia. Endotracheal tubes come in sizes 1.5, 2.0, 2.5, or 3.0 Cole. A wire stylet can be used to help place the tube more easily. Otoscopes with ear or nasal cones to help visualize the teeth and oral cavity of small animals. Intravenous catheters can be used as an endotracheal tube for smaller rodents if the stylet is removed. The catheters should be 12 to14 gauge. Umbilical tape is used to anchor the endotracheal tube snugly by tying it behind the rodents head. Cotton tipped applicators are used to clear out saliva and moisture and debris out of the rodent’s mouth. Explores and probes are used to examine the teeth their attachment structures. Hand pieces that are used in dental care for rodents are both high speed and low speed.
Tongue retractors are instruments with a flat blade that assist in moving the tongue away to view a certain area of interest. Mouth gags are used to keep the mouth open during the procedure. Most mouth gags are spring activated and can be adjusted to open wider with the turn of a screw device. Burs - such as diamond, carbide round, carbide tapered fissure, and white stone points - are used as a hard tissue device. Files/rasps are used to level abnormally occlusive teeth. Rasps cut on both the push and pull stroke and files only cut on the pull stroke. Molar or cheek teeth cutters may be modified hard tissue-nippers, pin and wire cutters pliers, or side cutting rogers. Elevators are used to work around the circumference of the tooth in the periodontal ligament and help in aiding the removal of the tooth. Blunted injection needles sized 18-25 can be used also for removal of teeth. Small animal extraction forceps are used to extract teeth that are loosened or diseased. With cheek teeth, a small 90-degree angled Halsted mosquito forceps with an angled root tip can be very useful for cheek teeth extractions.
Anesthesia is used to sedate the rodent and is available in two forms: injectable and inhalant. When injectable medications such as Ketamine and Acepromazine are used it usually entails a longer recovery time than inhalants. Inhalants such as Isoflurane and Sevoflurane are preferred for most anesthesia procedures. Anesthesia and oxygen may be delivered through the endotracheal tube.
After any dental procedure is done, continue at home dental care for the rodent. They should be given hard objects to gnaw on. Since their teeth continuously grow, the length will need to be monitored. The top incisors should be no more than 4mm long and the bottom incisors should be no longer than 7mm long. Most rodents will need to have them filed down every so often if gnawing isn’t helping. Cheek teeth also need to be checked because they can overgrow also. This is mainly to make sure the top and bottom teeth align so they can grind against each other.

Interest Since Childhood and Independent Study of Chinese in my Life
I had visions and dreams of attending school in China since I was 11 years old. It became almost an obsession for me and I elevated in my mind how perfect this experience would be. As a sophomore in high school, I achieved my goal! However, the dream of a perfect experience collided with the reality of the situation. In fact, the culture shock, the distance of being away from home, and the initial language barrier set that year up to be not only the most challenging and difficult time for me, but also one of the most fulfilling and transformative times of my life.
I arrived in Beijing, China in the fall of 2016, to study Chinese for 9 months at Beijing High School #80 through a United States Department of State scholarship program called the National Security Language Initiative for Youth (NSLI-Y). I was just barely 16 years old and knew very little Mandarin. However, what I lacked in knowledge I knew I would make up for with my energy and excitement. At this point, I believed that my goal was merely just getting to China. What I did not yet realize was that the real goal was to learn Mandarin and the journey was overcoming the obstacles it would take to become fluent.
I first had to adapt to living in a dorm room with another international student. My roommate was from Serbia and I struggled communicating with him but we managed to have limited conversations through broken Mandarin and English. We did not have much time to spend with one another due to the rigors of the curriculum. The class schedule was long and demanding. I attended Chinese language classes from 7 am to 5 pm. I had mandatory study time from 7 pm to 9:30 pm each week night. Despite the number of hours in class and studying, I found myself struggling. There was no English spoken in class and I found myself feeling overwhelmed in keeping up. I was not even being able to express how I felt emotionally due to the language barrier. I began to wonder if I made the right decision going there and this doubt caused me to consider lowering the goals I had set for myself. I was at a place that was not familiar to me. I found myself feeling sad, lonely, depressed and doubting my ability to succeed.
I had set a goal when I left for China to become fluent in Mandarin. The general coursework from our classes was HSK focused. HSK is the national Chinese fluency test for foreigners. There are a total of 6 levels and my goal was to pass HSK5. Level 5 would mean that I went from knowing only a few words on my arrival to an advanced level with a vocabulary in the thousands. This was a big goal but one I initially felt I could achieve. Every day I was assigned an average of 70 words to learn how to write and memorize. The following day I would be tested with a dictation. I failed every dictation for the first semester and it felt hopeless. To make the situation worse, the Chinese teachers ridiculed me, laughed at me, and repeatedly called me 'stupid'. I became their target. I kept being told by other students that this was a tactic to encourage me to do better, but it tore me down and took me to my lowest of lows. I poured into motivational videos and I worked on rebuilding myself. After several weeks, I emerged emotionally and mentally stronger. While going through this period, I had discovered a Chinese idiom that would become my battle cry. 'Don't fear slow, fear standing still'. This made a lasting impression on me and I would wake up each morning and repeat it to myself over and over again. It helped me to reshape my goal and I learned that the process of getting there would be much more important than the outcome. I now worked towards a daily goal of just increasing my knowledge of Mandarin. I knew that as long as I improved by even the smallest percent, then I would be moving forward! Going backwards was no longer an option to me. This shift in how I looked at my goal served as a light that steered me through these trials and tribulations.
I have applied this mindset to my life and I constantly strive to make those little improvements that make up a big difference over time. At the end of the nine months, I emerged with one of the highest scores on the HSK 5 and in the class. I found that through my journey in China, it was really the tough times that defined the type of person that I am. When I look back at all the pivotal moments in my school year there, I realize that I did not grow when I was feeling comfortable. Instead, it was always the struggles that I learned from and which created my successes. As I think about the struggles I had the first few months in not being able to understand the language, not being able to express myself to others, and getting lost in a city with over 22 million people, I take pride in what I was able to accomplish. After all of those moments had built up on each other, I remember climbing a mountain peak that looked over the bustling city of Beijing and thinking that not only am I navigating this country by myself but that I am navigating my life!
My travels through China impacted me in ways I never imagined. I came to understand that political, social and economic differences can be overcome by mutual respect for one another. I was welcomed in the home of a Chinese host family. I was able to experience a season of their lives and we got to know each other as people. I was immersed in their culture and this gave the time to have experiences that a traditional classroom setting will never provide. I had a twelve year old host brother and we learned from each other and created a great bond. In fact, we became brothers! My host father and mother showed me a surplus amount of love and opened up their lives to me. They were not only my parents but often served as mentors to me. They reinforced to me when I was at my lowest point that it is not intelligence that will bring success but consistent hard work and effort. I left feeling I made a lasting impact on those that I was around. I did this by creating relationships with them and valuing who they were as individuals and as a culture. I respected and learned my host countries traditions.

Myself Story about Learning and Education
“Bonjour.” This was the first word that I spoke upon arrival at L’Académie française, the Virginia Governor’s French Language Academy. Although this word is usually interpreted as a simple “hello,” it also possesses a deeper meaning, which is why it holds a special place in my heart. This seemingly ordinary word conveys much more than a greeting; its literal translation wishes upon others a good day. And living each day to its fullest was precisely how I intended to spend my time at the Academy.
While the beginning was indeed promising, I would soon realize that not everything was as picture-perfect as I had imagined it to be. When signing the oath committing myself to speak only French and avoid any form of English communication for three weeks, I was oblivious to the challenges that awaited me. This experience began as an internal struggle, as I often asked myself, “Why did I voluntarily place myself in this demanding full-immersion setting, away from my family, without a cell phone, and completely secluded from the rest of our world?”
As I questioned the wisdom of my choices, I was simultaneously reminded of an image of my younger self. The unwavering memory on which I was now reflecting involved my bewildered five-year-old self mounting the stairs of a school bus, surrounded by strangers speaking English, a language I was unable to comprehend at the time. After having immigrated to the United States from my native country, Israel, only a few days earlier, I was both eager and anxious for my first day of Kindergarten.
The constant replay of this vivid memory in my mind was now conjuring up the word “shalom,” a Hebrew word, the first one that I heard as my parents welcomed me into this world. Similar to 'bonjour,' this word also possesses a double meaning, but this time, along with “hello,” it means “goodbye.” This was precisely how I felt at that moment: farewell to my old life, hello to the beginning of a new chapter.
While I was yearning for this day since the moment I arrived in the United States, I was overwhelmed by the confusion circulating through my mind, and tears began flowing down my face. Since I felt as if I had just been ripped away from the only language, culture, community, and reality I had ever come to know, frustration and loneliness initially trumped my previous optimism.
My first year in the United States was extremely difficult. Learning a new language and immersing myself in a new culture required an immense amount of patience, perseverance, and resilience. Nonetheless, successfully overcoming these challenges left a powerful impact on the rest of my life, molding me into the multi-faceted individual that I have become. Following this experience, I promised my future self that I would forever continue to seek new adventures, never halt my curiosity for the meaning of words, and always embrace my unique cultural identity despite the obstacles that I would encounter along my journey.
This past summer, I fulfilled the promise I made to myself over a decade ago, by reliving an experience similar to the one I underwent upon my arrival in the United States. My three weeks at L’Académie française had turned into a life-changing experience. I was granted the opportunity to analyze French poems, learn Cameroonian dance, sing the national anthem of Côte d’Ivoire, immerse myself in making traditional crêpes, and admire the coexistence of cultures and religions in Senegal. The entire experience was an enlightening one, for each day, I was living a new adventure by enriching my mind with the study of the French language and culture. At the conclusion of L’Académie française, I said, “au revoir,” expressing not a final goodbye, but my intention to fulfill the literal translation of the phrase, “to be seen again.”

Forensic dentistry can be defined in many ways. One of the more elegant definitions is simply that forensic dentistry represents the overlap between the dental and the legal professions. This two-part series presents the field of forensic dentistry by outlining two of the major aspects of the profession: human identification and bite marks. This first paper examines the use of the human dentition and surrounding structures to enable the identification of found human remains. Conventional and novel techniques are presented.
Forensic Odontology is the correct care, review and evaluation of dental evidence, to be viewed in the interests of justice. The proof that can be extracted from the teeth, age (in children) and identity of the adult to whom the teeth can belong. The establishment of forensic odontology as a unique discipline has been attributed to Dr. Oscar Amoedo (considered the father of forensic odontology), who identified the victims of a fire accident in Paris, France
One of the examples of forensic dentistry was involving Agrippina, Roman emperor Nero's mother. In 49 B.C. Agrippina ordered the death of her rival Lollia Paulina who competed with her to be Emperor Claudius ' wife. Agrippina requested that the head of Lollia Paulina be seen as evidence of her death, but she was not sure that her opponent was dead until she found Lollia Paulina's distinctive discolored front teeth.

Teeth aren't fingerprints; from birth they're not necessarily special. When teeth erupt, each person does so differently. Teeth grow an amount of 4 micrometers per day, so an approximation of a rough age based on the teeth can be given. Ethnicity can be distinguished from teeth, too. bite mark Criminals have been known to leave bite mark impressions at the crime scene, whether it be in food, chewing gum or, more commonly, on the victim. When a bite mark is discovered, numerous steps should be taken. Once the mark has been sufficiently photographed, a saliva sample is taken from the area for potential DNA evidence. A Study of Drug Selection and Prescription Patterns Among Final Year Dental Students
ABSTRACT
The purpose of this study was to compare the choices and prescribing patterns of two supervised levels in teaching hospitals in Karachi.
The Cross Sectional questionnaire based study was conducted in May 2016. Students and House officers of Altamash Institute of Dental Medicine, Jinnah Medical and Dental College, Jinnah Postgraduate Medical Centre and Ziauddin College of Dentistry participated. The response rate was 72.6%. Half of the sample was of final Year Students and the other Half was of House Officers.
Results showed some similarities and few differences in choices and prescription patterns of Final Year students and House Officers.Out of 69 Final year Students 46.4% Prescribed NSAIDS, 59.4% prescribed for pain. Panadol was prescribed by 66.7% and 44.9%prescribed a combination of Amoxicillin and Metronidazole. According 71% of final year students they gave advice or supporting medication to counter the side effects of Drugs. Out of the Final Year Students, 53.6 % said that their choice of drugs was dependent upon Information from their Professor. From 69 House Officers 65.2%Prescribed NSAIDS, 58% prescribed for pain. Ibuprofen was prescribed by 39.1% and 69.6% prescribed a combination of Amoxicillin and Metronidazole. According to 69.6% of House Officers they gave advice or supporting medication to counter the side effects of Drugs. Out of the House Officers, 56.5% said that their choice of drugs was dependent upon Information from their Professor.
The choice and prescribing patterns of final year students and house officers seemed to be non-uniform. This strongly suggests that the training of both supervised levels is incomplete and a very high proportion of practioners-to-be are making uninformed decisions.
INTRODUCTION
An undergraduate course of Dentistry in Pakistan lasts for four years. It is followed by a training period of a year which is known as house job.After completion of House job, Dentists are licensed to work independently. Increasing evidence has been found, that dentists in most countries have inadequate pharmacological knowledge and that they make errors in drug prescription 1. This indicates that the training of dentists needs to improve during their undergraduate years or during house job so that they make good choices.Students start prescribing drugs in their final year, when they begin their clinics in the Departments of Surgery, Operative dentistry and Endodontics and then during House JobThere is reduced exposure to clinical pharmacology. Poor prescribing is widespread including overuse of medicines or underuse of effective medicines2.
The most common drugs used in dentistry include local anesthetics for procedures, non-steroidal anti-inflammatory drugs and antibiotics 3-6. The success of a treatment is dependent on proper choice and prescription patterns of these drugs.
It is the House officers and the Final Year Students that make the future workforce, it is therefore, necessary that their knowledge should be sufficient. Any deficiency in their final year should be overcome during the course of their house job 7.
Although in the past, the knowledge of dental students has been evaluated, this study covers a vast sample size from four public and private Dental institutions. It compares the practices of both supervised levels, which include the Final Year Students and the House Officers.
MATERIALS AND METHODS
The data was collected in May 2016, from four public and private institutions, in different areas of Karachi. A questionnaire with six close-ended questions was distributed amongst 190, Final Year Students and House officers. It was returned by 138 respondents out of which, 69 were house officers and the 69 were final year students. The results were stored in excel sheets and analyzed through SPSS software (version 24).
RESULTS
The most common response was regarded as the most significant. According to 32 (46.4%) Final Year Students and 45 (65.2%) House officers, Non- Steroidal Anti Inflammatory Drugs are the most common drugs prescribed in their clinical practice (Fig1). The most common condition for prescribing was pain according to 41 (59.4%) of Final Year Students and 40 House Officers (58%) (Figure2).According to 46 (66.7%) Final year students the most common NSAID prescribed in clinical practice is Paracetamol. Whereas, 27 (39.1%) House Officers prescribe Ibuprofen for pain (Figure 3).
When asked about the antibiotic of choice, 31 (44.9%) final year students and 48 (69.6%) House Officers most commonly prescribed a combination of Amoxicillin and Metronidazole (Figure 4). According to 49 (71%) final year students and 48 (69.6%) House Officers gave advice or supporting medication to counter the side effects of Drugs (Figure 5). Out of the Final year students, 37 (53.6 %) and 39 (56.5%) of House Officers the choice of drugs was dependent upon Information from their Professor. However, 18 (26.6%) Final Year Students confessed that their choice was influenced by medical Representatives and 5 (7.2%) students made their choices by asking friends or colleagues (Figure 6).
DISCUSSION
In the past, very few comparative studies have been conducted.This was a multicenter study in Karachi.It recorded choices from four different institutions present in different areas. Dental Students cover a course of pharmacology in Pakistan in their second year of study. They are under direct supervision of their teachers and faculty when writing prescriptions in their final year and house job.
It is expected that the standard of all Dental Students and House officers is similar as all dental institutions cover the same syllabus approved by the HEC and PMDC. However, differences in preferences of Final year students and House Officers were noted.
According to the present study the most common drug prescribed in clinical practice by both final year students and House officers were NSAIDS and the most common condition for which they prescribed was pain. This matched the results of previous studies conducted in India byJain et al 8. A significantly high proportion of final year students and House officers also prescribed for infection as was investigated by Gulzamen Alvarez et al the findings of in his study and Asad Mahmood et al in his study in Punjab, Pakistan1, 9.Removing pain is greatly important when treating dental patients, for both the patient and the clinician 10. Pain is a symptom that signals greater underlying problems. Major cause of pain is due to activation of sensory nocioceptors around the tooth by release of inflammatory mediators 4. Non-steroidal anti-inflammatory drugs (NSAIDs) are among the most widely prescribed analgesics for management of post-operative pain in dental patients 11.
In this study we analyzed that Final Year Students chose Paracetamol as the most common Painkiller advised. This is similar to the choice of Final year students participating in the study conducted by Guzman Alvarez et al.1 Most final year students probably choose this because they regard Panadol as the safest medication as compared to other painkillers available. In contrast, House officers in Karachi chose Ibuprofen as the most common painkiller followed by Paracetamol. Paracetamolhas analgesic and antipyretic actions but little or no anti-inflammatory properties 12.The preference of Ibuprofen in this study,over paracetamol could be due to its potent anti inflammatory activity.
Studies conducted in Scotland showed thatmost general dentists preferred to prescribe Amoxicillin, and a very high percentage of dentists also chose Metronidazole13. Similar results were found in studies done in Lahore. In this study majority of Final year Students and House Officersprescribed a combination of Metronidazole and Amoxicillin14.
Drugs used in dentistry have side effects. For example, Nsaids achieve their analgesic effect by inactivation of cyclooxygenase (COX) enzyme, that converts arachidonic acid into prostaglandins and leukotrienes 15,16,17. These COX inhibitors cause Gastro-Intestinal related side effects in patients. When giving such drugs it is effective to give supporting advice and medication, to patients, in order to counter such effects. Most Final Year students and House Officers had this habit as analyzed in this study.
Choices of Drugs, prescribed by House Officers and Final Year Students were dependent upon the Information provided by their Professors. It was encouraging to see a significant number of House Officers read literature for their choices. Same results were obtained by the study conducted by Asad Mahmood et al. However, it was alarming too see lots of students being misguided when making choices. Even though most were under guidance of their teacher a very high number was depending on Information from medical representatives. As commercialism is a dilemma in this stage it also poses a threat to the wellbeing of the patient. Many students were also depending on information from friends making their drug choices even more questionable.
CONCLUSION
The findings of this study showed slight discrepancies in the choices of Final Year Students and House Officers. This reinforces that even though both levels are supervised their standards are clearly not the same. Further focus needs to be done to uplift the level of training and study to produce better dentists in the field.

A Story about Myself, my Faith and Life
If you were to ask me why I believe in God, I would simply say that he saved me from myself. He showed me real love and worth, but I had to go through some things to get there first. I wish I could say that I could say that I have never strayed or walked away from faith, but then it wouldn’t be my testimony. Honestly, I don’t really believe my testimony started until last year. I live in a Christian household and I was raised in church. As a child I knew about God and Christianity, but didn’t quite understand fully. My childhood was a fairly normal one, but I was bullied quite a bit for my sensitivity. I was often lied to, stabbed in the back, pitied, and bullied by kids in my neighborhood and sometimes my brother. I said the sinner’s prayer around age nine, and was baptised around the age of eleven.
My friends in my hometown were the only ones keeping me sane at the time and then we had to move. When I first started in Tennessee I had no friends. I felt completely and utterly alone, unloved, and frankly worthless. It only escalated from there. By the time I had reached 7th grade, I was at the point of thinking that the world would be a better place if I wasn’t in it. I know see that I had fallen into depression. I would hide everything behind a mask and cry myself to sleep at night. I was overwhelmed by having people around me, who influenced me. I had a suicidal friend, a bully at school, and a brother, who often called me names such as useless and fat. I could never seem to make my parents proud and my dad was often gone. I didn’t know what to do. I thought about ending it, but I could never follow through.
As 7th grade ended, I was still slightly depressed, but I told myself that I would be more than just another suicide statistic on the scale. I told myself that I was a fighter. I still had lots of doubt about God. I figured that if he cared, he would talk to me. My disbelief led to being more easily tempted by Satan. I got into some things I shouldn’t have and tried to fill a void that only God could fill. 8th grade came and I just felt defeated and broken, yet I still had a mask of fake happiness on. When clubs started, I chose worship arts and the first day Mrs. Rains had us sit in silence and listen for God’s voice. We sat and I just listened. Then a verse had popped into my head. The verse was Ecclesiastes 3:17 “I said to myself, 'God will bring into judgment both the righteous and the wicked, for there will be a time for every activity, a time to judge every deed.'. I wasn’t sure if it was an actual verse, so when she asked if anything came to us, I said nothing. Later I looked up the verse and was just awestruck. I didn’t know what the verse meant, but God had finally said something. This made me really reevaluate my faith and I asked God’s forgiveness.
Later in the year someone had told me that they wished I would die. That night I was broken all over again and cried, but while I was crying I was also at a point where I was so broken that all of my walls were down and I heard God again. He showed me Ecclesiastes 7:17 “Do not be overwicked, and do not be a fool-- why die before your time?”. That hit me hard. I really had felt an overwhelming sense of love, acceptance, and self worth for the first time in a very long time. Over time after that I really started to work on myself. I still have a very low self esteem, but God is still working on me. One song that God has really reminded me of is called Blessings by Laura Story. Every once and a while, God will just fill me with an overwhelming feeling of his love to remind me that I am not worthless, I am priceless in His eyes. Whenever I feel that love I start crying on the spot because I had felt worthless for so long, that whenever someone tells me that I am not worthless or when my dad says that he is proud of me I can’t help it. God has truly saved me from myself, and if he hadn’t said something when he did, I might not be here today. God has healed my depression. I am still broken, but aren’t we all? We are all just one group of broken people made whole by God. That is my testimony.

Story about myself from School to Today
My ambition throughout school was to become a social worker, I knew from a young age that I wanted to help others as much as I possibly could and I was certain healthcare was the career path for me. I started my first job at 18 as a support worker, my client was a schizophrenic with borderline personality disorder, and my other client was autistic. After being in the job for a few months I quickly decided that the mental health sector was where I wanted to be as I enjoyed my job so much, I was intrigued and wanted to continue learning more. I learnt that I was able to maintain a professional manner inside my job, and I had the ability to switch from work mode to home life upon leaving my shift. I feel this is necessary in the mental health field. Shortly after starting my job I enrolled myself in to college and completed my level 3 health and social care qualification, my ambition was now to become a mental health nurse, as it still is today five years on.
One in four people in the United Kingdom will experience a mental health problem at some stage in their life. Mental health issues can happen to anyone from all different walks of life, no matter who you are. Just short of a year after my son was born I soon realized this myself. I've always seen myself as a confident driven woman who gets an idea in her head and works for what she wants until she gets it, and getting pregnant with my son was unexpected but it wasn't going to stop me achieving my goals that I had set for myself. This was until postpartum depression hit me smack bang in the face and I completely lost who I was. I was too proud to ask for help or advice of anyone close to me so I suffered in silence and was isolated for a while trying to get my head around how this was happening to me. My postpartum deteriorated rapidly until I finally couldn't take it and went to the doctors for help, I couldn't understand how I could be amazing at my job supporting others who were suffering but couldn't help myself. I learned the hard way how to take care of myself, an illness I had no clue about forced its self upon me to teach me a valuable lesson. I had to take care of my own mental health before I could help anyone around me.
Reluctantly taking antidepressants from my doctor I slowly began to feel more like me again and taking back control over my life, I took some time off work and spent most days getting out of the house doing activity's with my son as I felt it would take my mind of being in that dark place I was in. Baby groups with the other mum's really helped me as I didn't feel so alone and getting out with my friends made me come out of my shell that I had became hidden in. I feel that my knowledge from work but also my personal knowledge after suffering a mental illness myself has made me confident in my approach to delivering the best quality of care that I am capable of giving to individuals who need it.
My time spent working in healthcare has been beneficial to me as I feel that I've experienced working in numerous healthcare settings with various individuals with various health issues. I have spent time caring for the elderly, individuals in the community, the homeless sector, individuals struggling with addiction and individuals in secure units. I am now confident in giving medication and first aid if required. I have been trained in physical interventions and have worked with behavioral specialists during my training. After taking some time out of my studies to take care of my son and to build my work experience up I now feel that I am ready to continue in university to achieve my degree, and to progress my career as a mental health nurse in the prison sector.

How can people treat others they don't know so horribly when they have done absolutely nothing wrong? In this war, how do soldiers do what they do without feeling terrible about it after? Today I helped a woman with a baby she was carrying on her back. Her baby must have done something to catch one of the guard’s attention. I am not sure what but the guard was definitely not happy. I know this because his bulldog-face raised his gun to shoot her or at least threaten to. I have never been able to behave “morally” as an adult but in this setting I did, I keep asking myself how could I stand up for this woman and her baby when I can't even stand up for myself. But I guess, most of the time it was some fatherly instinct I didn't know I was capable of.
We left Afghanistan for good this time. I feel this sudden sadness. I don't know how to explain the way I feel. I know I can't do anything about the war, but I had to do something to help this woman. Before I saved her I was talking to Amir when the officer let us pass. He told me there is no shame in war, but what does he mean? I told Amir he had to be wrong. I told him he’s wrong. War doesn’t negate decency. It demands it, even more than in times of peace. I knew Amir would follow the way I am. He always does; he is a follower, not a leader. I don't want him to follow me, I don't want him to be the way I am, I want him to be himself, not me. I'm not a good man. I can't stand up for myself and no matter what happens to Amir I want him to be able to stand up for himself. I may look like I am a fighter but I am not. I do helpful things for others and I try to help myself as well. I built an orphanage and paid for Hassan's surgery but I still do not consider myself a man. I only do those things so I keep my image.
I do love Amir, however sometimes I worry about him. I know he wants to study english, I am proud he is following that. He is a good writer, he could be if he tried. He can do anything he puts his mind to, he just has to try. I think about going to America and if it is the right decision but the Russians invaded Afghanistan and executions became common, Amir and I had to leave Afghanistan, the war was destroying our home and I didnt want Amir or I to be there when it got to our part of afghanistan. We’re in America now, in California. But it seems I misunderstood the “American way” ; it insults the status I am in at this point as I am working at a gas station, Amir and I aren't in a high social status. Amir is adapting better than I am. Amir thinks about the good side of things. He is a glass half full kind of person. He thinks living in America is a way for him to start over and absolve himself from the sins against Hassan in Afghanistan.
I am glad I helped that Afghan woman because it proved to Amir and myself that if I can stand up for other people maybe I can stand up for myself. And now I am hoping Amir follows that example instead of the one I have set before then.

'Without continual growth and progress, such words as improvement, achievement, and success have no meaning.” says Benjamin Franklin. Occasionally, we need to grow in order to better ourselves.
I would give myself a C in mood management. The reason I would give myself a C for mood management is because I don’t have patience for people that aren’t punctual with time. Nobody likes waiting, wondering if the person is forgetting about their plans. Last week, I was annoyed with my boyfriend because we agreed on a set time to go out to eat, but he was an hour late because he forgot. I think it’s important to know that when you’re late, you don’t just waste other people’s time, you steal all their time. In another related incident, I had to set up for a family event and my cousin was late to pick me up. My cousin knew we had to be at the event by 2pm, but she thought it was okay to be late. It’s definitely difficult to stay calm when somebody isn’t punctual with time.
Besides giving myself a C in mood management, I would give myself a B in self motivation. Erratically, people make goals that they don’t accomplish because they don’t push themselves. Last week, I had two papers to write and they were both due on the same day. I pushed myself to finish my papers a few days early because I didn’t want to feel stressed. Achieving your goals without needing anybody to motivate you is a sign of self motivation. In another related occurrence, a couple months ago I started to motivate myself to start my day early. Self motivation is when you are the only one that is motivating yourself to achieve your goals.
In addition to self motivation, I deserve a B in people skills. Commonly, people skills is defined as communicating and team work. Last year, I had a project to work on with my coworker for my boss’s wedding. We were able to get everything done in a timely manner. When you work with others, you brainstorm, create, and share your thoughts. In another related incident, last week I helped my coworker when she was struggling on a task. There was a lot of frustration, but we were able to finish together. People skills are important in a workplace in order for something to succeed.
Continual growth and improvement begins with “I”. I will see everyday as an opportunity to become a better person. I will also improve on controlling my emotions. Ultimately, I know I will never be perfect, but I will always strive to become a better me.
